Discharge Coordinator

Job Summary The role of the discharge Co-ordinator is to provide support to wards, attend board rounds and ensure that all patients are discharged safely with the care they require. To be able to escalate any delays in care or discharge planning To liaise with other team members for patients from care homes and patients that are approaching end of life SECTION 2- KNOWLEDGE AND SKILLS The post holder needs to have effective communication skills as they will be speaking with patients, families, next of kin and other members of the MDT within the acute trust and also in the community The post holder will need an awareness of what services are available in the community to support safe discharges and will need to complete referrals to these services The post holder will need to work well within the wider IUCT acute team The post holder will need good IT skills as we use acute and Tameside council data SECTION 3- EFFORT AND ENVIRONMENTInformation that could be included might be: IUCT Acute provides a seven day cover to the acute wards within the hours of 08.00-16.00 To be able to provide daily electronic updates onto data systems currently in use To be able to speak with distressed patients and families and be caring and compassionate at all times
