Job summary

Are you a health visitor with a minimum of 3 years experience ready for a new challenge as a Public Health Team Lead? Southend City Council Children, Young People and Families Public Health (CYP&FPH) Service would love for you to come and work in our vibrant community, supporting children under 5 and their families.

We are looking to recruit an enthusiastic and motivatedpractitioner with initiative and drive. Our 0-19 service delivers high qualitypublic health nursing, that promotes and improves health to children,young people and their families in Southend-On-Sea. You will need to haveexcellent communication and organisational skills, be willing to facilitate andsupport change in service delivery, have the ability to work autonomously andas part of an integrated skill mix team.

Main duties of the job

The Health Visiting Team is looking for a strong, enthusiastic health visitor to lead a Team in the delivery of the Health Child programme and support the team in managing their safeguarding caseloads. As a Team Lead you will have your own caseload to manage alongside team management responsibilities. You will be required to lead the delivery of the Healthy Child Programme and promote the health of all children and young people, especially those from disadvantaged groups, to ensure they are offered the opportunity to achieve optimum development.

To Identify, prioritise, develop and implement programmes of care to meet individual and local needs. This will be achieved through evidence based assessments, effective care planning and implementation of a range of support packages for individual families, targeting the most vulnerable families and groups utilising best available evidence.

Work in accordance to the commissioned service specification, to ensure the 5 mandated reviews are completed within required timescales, services provided are evidence based practice.

Job responsibilities

To manage a designated Health Visiting Team. Completing the teams 1-2-1s and annual appraisals.

To manage the safeguarding dashboard with support from the Professional Leads, ensuring your teams figures are cleansed and workloads managed.

To manage your teams sickness, annual leave and proformance/capability with support form your Professional Lead and Human resources.

To Lead the weekly Work allocation meetings feeding down information to your team from the Public Health Team Leader meetings.

To lead the delivery of the Healthy Child Programme andensure the 4, 5, 6, model is implemented in practice, ensuring services areprovided within the tiered model approach Building Community Capacity,Universal, Targeted and Specialist.

To promote the health of all children and young people,especially those from disadvantaged groups, to ensure they are offered theopportunity to achieve optimum development.

To clinically interpret information, analyse and identifyappropriate individual plans of care in line with Universal, Targeted and Specialist cases within the Health Visiting caseload.

To appropriately delegate and supervise workloadwithin the skill mix team in line with service operating procedures and competencyframeworks.

To provide expert advice, support and interventions to allfamilies with children in the first years of life.

To identify the needs of individual children, parents andfamilies (including safeguarding needs) and refer or direct them to existinglocal services, thereby promoting early intervention.

To operate within Children & Young People Services ChildSafeguarding Policies, Standards and Procedures, recognising and takingappropriate action in relation to children where there is suspicion of abuse.

To participate in strategy meetings, case conferences,providing reports as required and to have clinical and safeguarding supervisionas per policy for Safeguarding children.

Liaise and maintain good working relationships withstatutory and voluntary agencies whilst making referrals and consulting asnecessary.

Qualifications

Essential

First Level Nurse Registration or midwife registration Specialist Community Practitioner qualification in Health Visiting. Minimum of 3 years experience as a SCPHN health visitor Evidence of continuous professional development Mentorship course/CPT or equivalent Comprehensive knowledge community based health services including national policy
