Job summary

We are seeking a dynamic, highly motivatedindividual to become part of our Enhanced Home Care Support Team. The triage roleis an integral part of the team and is the first contact with the nursing andresidential home staff when a patient is unwell, or they are requiring advice, manyof whom are frail, elderly with long term conditions and complex health needs.

The nurse undertaking the triage will be requiredto undertake a full assessment of the presenting complaint over the telephone,whilst also confirming the patients past medical history and other contributingfactors, thus requiring a vast clinical knowledge.

The role will involve triaging the patientsneeds with health care professionals, social workers, and care home staff, makingautonomous clinical decisions to ensure best practice for all patient care and providingadvice and support as required.

The role requires coordinating staff in the team asnecessary to ensure visits / assessments are undertaken by the most appropriateprofessional as clinically indicated, to ensure high quality care that isappropriate to the patients needs and is delivered in a timely and proactivemanner.

The role requires a multi-disciplinary team approach,and the successful candidate must be able to demonstrate excellentcommunication skills, flexibility, and a commitment to caring for people,whilst contributing to the development of the service.

Main duties of the job

RGN/RN Adult Diploma or Degree on the NMCRegister Ability to work as a team player Work as an autonomouspractitioner Provide high quality complex clinical triage usingknowledge, skills, critical thinking, and professional judgement Identifylife threatening symptoms and act appropriately. Duties and Responsibilities

Clinical

Provide a high-quality telephone triage assessment of all contacts to the triage service. Identify life threatening symptoms and act appropriately. Provide self-care advice and management of presenting symptoms, when appropriate. Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.

To establish and maintain effective communication with individuals, groups, and communities in complex and potentially challenging situations.

Maintain excellent communications with all team members including care home staff and other professionals Work collaboratively with other professional and agencies to ensure patients needs are met. Involve patients and carers in the planning and delivery of care. Maintain accurate records in accordance with NMC Standards. To identify skills and knowledge to address individuals training needs by undertaking personal development reviews and personal development plans within the KSF framework. To participate in the health needs assessment of the practice population to identify areas of unmet need. Ability to work without direct supervision and to solve problems. Work to a flexible rota to maintain the needs of the service.

Managerial/Leadership

Effectively manage the triage and coordination session for the service, planning, prioritising, and coordinating the workload across all aspects of the service. Work in partnership with other clinicians on duty to maintain effective communication about workload, case mix and outcomes. Liaise with colleagues at other locations to utilise resources effectively and efficiently. Always promote a safe and effective service. Support other staff as necessary. Monitor workload, to ensure accredited standards are met throughout the session. Adhere to Trust policy and procedures. Support and contribute to the future development of the service. Work under the direction of the Service Lead in providing triage services to patients within care homes. To provide accurate timely information to the identified administrator regarding annual leave, expenses and study leave. To report any concerns with regard to poor practice or concerns of staff members to the Service Lead. Support the effective use of physical and financial resources. Ordering supplies and equipment. To ensure as far as possible that the working environment is safe, adhering to the Lone Worker policy. To provide leadership and support by promoting professional development and encourage clinical supervision. Supply required activity data related to patient care.

Clinical Governance

To support the clinical governance agenda by benchmarking best practice, identify any inequalities in service delivery. Ensure that practice is evidence based through undertaking audit and critically appraising literature. To contribute to the development of and implementation of policies, procedures, and guidelines relevant to own area of work to ensure best practice. In-depth knowledge of the legal, ethical, moral and accountability issues related to Community Nursing. Adhere to the NMC Standards:- Professional Code of Conduct, Scope of Professional Practice and Administrations of Medicines records and Record keeping. Support the organisations public health agenda. Maintain a professional portfolio that demonstrates evidence of on-going professional development. Identify any limitations/gaps in professional or clinical knowledge. Access and promote clinical supervision within the team Promote peoples equality, diversity, and rights.

Teaching and Learning

To ensure own continued professional development. To undertake mandatory training and other training that is specific for the post. Facilitate the placement of pre and post registration nursing students. Support the development of the nursing team by teaching and assessing other member of the team including nursing auxiliaries. Teaching of patients/carers and other professionals to support patients to maintain their optimum level of independence. Promote lifelong learning with the nursing team. Act as practice teacher to specialist practitioner students (NMC Standard 2006)

Records Management, Confidentiality and Security of Information

The post holder will be expected to:
