Job summary

We are looking for anenthusiastic, dynamic and forward-thinking individual with excellent,organisational, communication and clinical skills to join our Urgent CommunityResponse service. You will be required to work as an autonomous practitioner andbe a team player as part of larger multidisciplinary team. Our UCR service workswith patients in their homes across the Warrington Borough to avoid hospitaladmissions and support those being discharged home.

The base is currently Irwell Road Clinic UCR, this will be subject to change.

Our multidisciplinary teamwork flexibly to provide a 7-day service, 365 days a year between 8am and 8pm.

-We deliver rapid response topeople in the community who are experiencing acute episodes of ill-health andrequire complex management in order to prevent hospital admission and supporttimely hospital discharges. You will have the responsibility for proactivelymonitoring and managing patients to ensure the best outcomes.

Main duties of the job

To act as a senior clinical professionalwithin the Rapid Response service, as an integrated health and social care teamproviding an urgent rapid response to Warrington residents, experiencing ahealth crisis. The aim is to support people to remain at home rather than beadmitted to hospital. Liaising with patients the team and colleagues across thewider health and social care environment including NWAS the postholder willpromote, maintain and enhance effective multidisciplinary patient care.

To act as an independent autonomouspractitioner to provide specialist treatment and advice to patients and staffwhich will contribute to the assessment, diagnosis, care and treatment ofpatients at home preventing unnecessary hospital admission. All referrals willbe triaged immediately and responded to within two hours, with referrals fromNWAS prioritised.

Possess and demonstrateextended clinical skills in the assessment, diagnosis, treatment and managementof a wide range of patient care needs. Professional autonomy and clinicaljudgement will be exercised in managing, referring and signposting patients onto wider community services and assets.

Job responsibilities

Participatein and undertake triage to ensure smooth progress of patients through thesystem.

Comprehensivelyassess patients with complex physical, mental and social care needs usingadvanced clinical examination skills, detailed history taking and biometricmonitoring and evaluation skills.

Receivepatients with undifferentiated and undiagnosed health care problems andinitiate and conduct clinical investigations as appropriate.

Analysesigns and symptoms, laboratory tests and other measures of function, informulating a diagnosis.

Performa medication review and prescribe and change medications as required inaccordance with national and local guidance.

Prescribe following a full holistic assessmentand diagnosis for an acute condition.

Exerciseindependent and delegated prescribing rights in accordance with current andlegal and CCG criteria, and within the clinical situation.

Have an understanding of the combined impact ofall patients' conditions on their health and social care needs in order toimprove health outcomes and quality of life.

Develop short-term individualised treatment andmanagement plans using evidence based practice and proactively monitor patientsand review care plans for up to 72 hours.

Use advanced skills and expert knowledge toidentify subtle changes in condition during this period and take appropriateaction.

Exercise a high degree of clinical autonomy andprofessional judgement, treating patients independently within the service,taking an asset based approach in supporting and managing the handover ofpatient care to other community based services.

Identify and plan preventative measures andanticipatory care needs.

Identifyat risk patients and manage appropriately in accordance with national and localpolicy and procedures.

Act asan advocate for the patient at all times to ensure their individual needs,preferences and choices are taken into consideration.

Professional

Utilisingown professional knowledge and skills, act as a resource for other members ofthe team.
