You will be part of our Rapid Response team facilitating and delivering intermediate domiciliary care for clients being discharged from hospital and other intermediate care settings to their own homes.

**Shift pattern**

Salary payment is based upon completing **7x 6 hours shifts** including alternate weekend working
- 07:00am - 12:00pm
- 12:00pm - 18:00pm
- 18:00pm - 23:00pm

**Essentials**
- 12 months of paid experience within the care industry
- Full driving licence with access to your own vehicle that you are happy to use for work purposes

**Job Types**: Full-time, Permanent

**Salary**: £22,500.00-£24,500.00 per year
