Job summary

Imp PCN is seeking a highlymotivated enthusiastic Senior Clinical Pharmacist to join the team and be part of the growth anddevelopment of Imp PCN in this changing landscape of primary care.

The post holder will oversee the clinical pharmacy provision for Imp PCN,which currently includes practice-employed clinical pharmacists and pharmacytechnicians, as well as subcontracted clinical pharmacists and pharmacytechnicians working within the PCN practices or remotely.

The post-holder will work with the PCN Clinical Directors and the PCN Pharmacy working group to provide efficient clinical pharmacy provision across the entire PCN comprising of 9 GP practices.

Main duties of the job

Provide leadership on medicines-related issues for the PCN and mentor PCNclinical pharmacists, pharmacy technicians, and prescribing support staff. Support integration of general practice with wider healthcare systems toimprove patient outcomes, ensure better access to healthcare, and managegeneral practice workload. Lead and mentor the team, provide support, and manage employee relationsissues for direct reports. Develop and manage new services based on newmedicines or NICE guidance, such as care pathways for new oral anticoagulantsfor stroke prevention in atrial fibrillation. Manage a caseload and run clinics for long-termconditions, prescribing as an independent prescriber for relevant conditions. Review medication needs and support patients inoptimising their medicine use. Conductclinical medication reviews, making changes within the scope of practice orreferring to other professionals as needed. Home visits may be required. Manage caseloads for common/minor/self-limiting ailments, referring toGPs or other professionals as appropriate. Provide clinics for patients with questions or concernsabout their medicines, either face-to-face, via telephone, or online. Ensure PCN compliance with CQC standards related tomedicines. Provide extended hours services as part of the wider network, includingpatient-facing and telephone consultations.

Job responsibilities

The role of the Senior PCN Clinical Pharmacist is to manage all clinical pharmacy services and medicines optimisation within the IMP Primary Care Network, consisting of nine member practices. The post holder will coordinate pharmacists across different providers to deliver a comprehensive service for all member practices.

Responsibilities include clinical supervision of all pharmacists and pharmacy technicians, ensuring robust clinical supervision that meets CQC requirements. The post holder will provide expertise in clinical medicines optimisation, including structured medication reviews with direct patient contact, and contribute to:

Management of long-term conditions Minor illnesses Medicines management on transfer of care Reviewing systems for safer prescribing Contributing to repeat prescription authorisations and reauthorisation Actioning acute prescription requests Addressing public health and social care needs of patients Contributing to QOF and locally commissioned quality improvement schemes Conducting clinical audits

The post holder will also contribute to service development and delivery, involving regular engagement with audit and QIP, with outputs presented to the wider PCN.

CLINICAL AND SERVICE DELIVERY RESPONSIBILITIES 1. Leadership and Management

Provide leadership on medicines-related issues for the PCN and mentor PCN clinical pharmacists, pharmacy technicians, and prescribing support staff.

Support integration of general practice with wider healthcare systems to improve patient outcomes, ensure better access to healthcare, and manage general practice workload.

Lead on person-centred medicines optimisation, quality improvement, and antimicrobial stewardship.

Understand and implement pharmacy governance within the workplace.

Lead and mentor the team, provide support, and manage employee relations issues for direct reports.

Promote diversity and equality in people management techniques.

2. Service development Develop and manage new services based on new medicines or NICE guidance, such as care pathways for new oral anticoagulants for stroke prevention in atrial fibrillation.Provide specialist input in developing additional facing long term condition clinics

Manage a caseload and run clinics for long-term conditions, prescribing as an independent prescriber for relevant conditions.

Review medication needs and support patients in optimising their medicine use.

4.Patient facing clinical medication review

Conduct clinical medication reviews, making changes within the scope of practice or referring to other professionals as needed. Home visits may be required.

5. Patient facing domiciliary/home visits

Manage a caseload of vulnerable housebound patients, supporting PCN Clinical Pharmacists and Pharmacy Technicians with their caseloads.

6. Management of common/minor/self- limiting ailments

Manage caseloads for common/minor/self-limiting ailments, referring to GPs or other professionals as appropriate.

7. Clinical Decision Making

Manage caseloads for diagnosing long-term or acute conditions, referring to GPs or other professionals as needed.

8.Patient facing medicines support

Provide clinics for patients with questions or concerns about their medicines, either face-to-face, via telephone, or online.

9.Medicine information to practice staff and patients

Answer medicine-related inquiries from GPs, practice staff, healthcare teams, and patients.

Reconcile medicines post-hospital discharge, managing changes and ensuring continuity of care.

17.Telephone Triage

Refer patients to appropriate healthcare professionals based on their needs.

18. Repeat prescribing

Ensure robust repeat prescribing policies across the PCN and manage medication requests and reauthorisations.

19. Information Management

Analyse and present medicines data to support decision-making and produce timely reports.

20. Medicines Quality Improvement programmes

Conduct clinical audits and implement medicines optimisation initiatives to improve prescribing behaviour and patient outcomes.

21. Medicines safety

Implement changes based on MHRA alerts and other guidance, providing specialised advice on risk management.

22. Collaborative Working Arrangements

Collaborate with PCN Clinical Directors, Pharmacy Taskforce Group, and other stakeholders to ensure consistent patient care.

Registration with General Pharmaceutical Council Professional knowledge acquired through vocational masters degree in pharmacy (4 years) + 1 year pre-registration training Extensive knowledge of medicines optimisation, acquired through post graduate clinical pharmacy diploma or equivalent experience or training. Holds or working towards an independent prescribing qualification
