Position Overview

Imp Federation Ltd is in search of a dedicated and skilled Lead Clinical Pharmacist to become an integral part of our team, contributing to the advancement and evolution of our services in the dynamic realm of primary healthcare.

The successful candidate will be responsible for overseeing the clinical pharmacy services within our network, which encompasses both in-house clinical pharmacists and pharmacy technicians, as well as external clinical pharmacy professionals operating within our network or remotely.

The Lead Clinical Pharmacist will collaborate closely with the Clinical Directors and the Pharmacy Working Group to ensure effective clinical pharmacy services across our network of multiple GP practices.

Key Responsibilities

  • Provide strategic leadership on medication-related matters for the network and mentor clinical pharmacists, pharmacy technicians, and prescribing support personnel.
  • Facilitate the integration of general practice with broader healthcare systems to enhance patient outcomes, improve access to healthcare, and manage the workload within general practice.
  • Lead and mentor the pharmacy team, offering support and addressing employee relations matters for direct reports.
  • Develop and implement new services based on emerging medications or clinical guidelines, such as care pathways for new anticoagulants aimed at stroke prevention.
  • Manage a patient caseload and conduct clinics for chronic conditions, prescribing independently for relevant health issues.
  • Assess medication requirements and assist patients in optimizing their medication usage.
  • Conduct clinical medication assessments, making necessary adjustments within the scope of practice or referring to other healthcare professionals as required. Home visits may be part of this role.
  • Handle cases for common, minor, or self-limiting conditions, referring to GPs or other professionals as appropriate.
  • Provide consultations for patients with inquiries or concerns regarding their medications, available in-person, via phone, or online.
  • Ensure compliance with regulatory standards related to medications within the network.
  • Offer extended hours services as part of the broader network, including patient-facing and telephonic consultations.

Clinical and Service Delivery Responsibilities

The Lead Clinical Pharmacist will manage all clinical pharmacy services and medication optimization within the network, coordinating pharmacists across various providers to deliver a comprehensive service for all member practices.

Responsibilities include:

  • Providing clinical supervision to all pharmacists and pharmacy technicians, ensuring compliance with regulatory requirements.
  • Offering expertise in clinical medication optimization, including structured medication reviews with direct patient interaction.
  • Contributing to the management of chronic conditions, minor illnesses, and medication management during transitions of care.
  • Reviewing systems for safer prescribing and participating in quality improvement initiatives.
  • Conducting clinical audits and engaging in service development and delivery.

Financial Responsibilities

Ensure cost-effective prescribing practices, minimize waste, and maximize reimbursements for the network and practices.

Training and Education Responsibilities

Facilitate ongoing training and education for pharmacy staff to enhance their skills and knowledge.

Research and Development

Identify gaps in the evidence base, evaluate literature, and apply research findings to practice.
