Main duties of the job

You will have a commitment to providing holistic care to patients in their own homes, be able to work on your own initiative and work well within a team. Key clinical skills include complex wound care, bladder and bowel management, palliative and end of life care as well as medication administration.

Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Working within our Community Nursing Team you will support, assess and treat patients with a variety of health needs, delivering high quality care in line with evidence based practice.

Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.

Job responsibilities

Working in collaboration with the multi-disciplinary team and colleagues to:-

- Promote independence and support patient centred care planning.- Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).- Work autonomously, in line with your scope of practice, competence and policies.- Handover patient information to the wider team, document patient information effectively.- Escalation of patients needs as appropriate.

Work Experience

Essential

Relevant experience/placements to the role including assessment and management of long term conditions, palliative care conditions and wound care. Ability to manage time and caseload effectively. Supporting service users, families and carers in dealing with people with nursing needs. Working in a multi-disciplinary team.

Desirable

Experience in the delivery of healthcare. Relevant placements including surgical, medical, palliative care.
