Role Summary

The role holder will work in the Community to provide holistic assessments with patients in their own home. Along with supporting the organisation and running of the strength and balance classes. You will be working with people with a wide range of disabilities and acute or long term conditions. The role holder develops, delivers, monitors and evaluates the outcome of a therapy plan based on their assessment. Managing their own caseload whilst delegating and supporting the Band 3 clinical support workers.

We have the exciting opportunity to appoint a Band 4 Assistant Practitioner to join our enthusiastic Community Therapy Team for hours a week, based in Ilfracombe at the Tyrrell Hospital.

We are looking for a practical, proactive and passionate individual with good communication skills. We are looking for an applicant with experience of working in a care or rehabilitation setting who is eager to work in our multidisciplinary Community Therapy team.

The role addresses health promotion, rehabilitation, equipment provision and urgent, unplanned work to prevent hospital admission and support discharge from the acute setting. The post holder will be an integral part of the team in terms of service delivery and development. We positively encourage joint working between members of the community therapy, and broader health & social care team.

We are happy to consider Band 3 applicants for recruitment under Band 3 salary, terms and conditions who would be keen to progress via the Trainee Assistant Practitioner programme.
