Role Summary

To provide day to day management to an allocated team of staff, to provide advanced knowledge and act as a resource to guide and lead practice to more junior staff and to the MDT. To undertake a team leader role responding to the Service Lead/ Health & Social Care Community Services Manager in planning, co-ordinating, delivering and evaluating the service provided. Provide highly specialist assessment, diagnosis, treatment and advice to patients and their carers, including a wide range of acute, chronic and terminal care cases, many having highly complex disabilities and/or presentations. Lead the Clinical Specialty providing leadership, development and governance in relation to the clinical area and supporting other Clinicians regarding patient treatment and management. Be part of an interdependent, multidisciplinary team ensuring that therapy input is integrated into the patient's overall care plan. Work with the Service manager/professional lead to lead and develop the service in line with patient need and trust wide developments and to help provide an equitable service across the Trust.

To be responsible for the management of the Community Rehabilitation Therapy team that covers Ilfracombe, Braunton, Lynton and surrounding areas. This team is comprised of Occupational Therapists, Physiotherapists, Assistant Practitioners Clinical Support Workers and Coordinators. With Advanced Practitioners covering Physiotherapy and Occupational Therapy already within the team this exciting leadership opportunity is available to either profession.

You will be required to provide leadership that supports both the performance and governance arrangements of the Community Rehabilitation Team. Responsibilities include caseload management, budget management, staff management/development and service evaluation and development.

The Rehabilitation team is part of an integrated Health and Social Care team so the you will work closely with the Nursing and Social Care teams and link with the acute setting to ensure effective safe patient care and promote patient flow.

Working Pattern: Monday to Friday, On-call and weekend working rotas are in addition to normal working hours. These rotas are currently based in North Devon District Hospital (NDDH) and may change to include other locations as services develop.
