Role Summary

The individual in this role will provide comprehensive assessments to patients in their homes, as well as assist in organizing strength and balance classes. The role involves working with individuals with various disabilities and conditions. Responsibilities include creating, implementing, monitoring, and assessing therapy plans based on evaluations. In addition, managing a caseload independently and providing support to clinical support workers.

About the Role

We have a great opportunity for a Band 4 Assistant Practitioner to join our dynamic Community Therapy Team in Ilfracombe at Tyrrell Hospital. We are seeking a proactive, hands-on individual with strong communication skills and previous experience in a care or rehabilitation environment. The position focuses on health promotion, rehabilitation, equipment provision, and unplanned interventions to prevent hospitalizations and facilitate discharges from acute care settings. Collaboration with other team members and promoting joint work between community therapy and broader healthcare professionals is strongly encouraged.

We welcome applicants from Band 3 who are interested in advancing through the Trainee Assistant Practitioner program.

Working Pattern: Hours per week, Monday to Friday - -
