Job summary

The main focus of this role will be extracting and interpreting relevant data from patients' clinical notes and inputting that information in a timely manner onto The National Hip Fracture Database (NHFD) and Fracture Liaison Service Database (FLSDB), which are both part of the National Falls and Fragility Fracture Audit Programme (FFFAP). The information allows the team to measure performance against national benchmarks and identify any specific areas needed for improvement.

The post-holder will work alongside and be supported by the Orthopaedic / Elderly Trauma Nursing team and the Fracture Liaison Service.

The ideal candidate will be able to work well independently and will have excellent communication skills tobe able to engage and build strong relationships with both Clinical and Clerical colleagues ranging in seniority.

We are looking for an individual who pays meticulous attention to detail with great organisational skills.

The successful candidate will have good IT skills, as the ability to accurately maintain computerised records on our systems and spreadsheets is essential.

Main duties of the job

The post holder will be responsible for collating information in relation to the clinicalpathway and outcomes for patients admitted to the Trust with fragility fractures. This will mainly be patients with hip and femoral fractures but may include other patients admitted with fragility fractures who have been reviewed by the Geriatric Trauma Team.

The post holder will work closely with the Orthogeriatric team to ensure the accurate andtimely collection, input, validation and submission of data to the National Falls and FragilityFracture Audit programmes (NHFD - National Hip fracture Database and FLSDB - FractureLiaison Service Database) will co-ordinate the booking of follow-up appointments when appropriate.

The Post holder will help in the production of reports from the data that will contribute to the department's clinical performance reporting, to highlight areas of good practice and where improvements are needed to ensure patient care meets the criteria and performanceindices for best practice, including monthly reports highlighting reasons patients have notmet the best practice criteria.

About us

North Bristol NHS trust Fracture Liaison Service identifies around 2000 patients a year who have sustained a fragility fracture, with around 600 of these patients admitted directly because of fragility hip and femoral fractures.

Our small, friendly teams of specialist nurses and administrators work together with the Consultant Geriatricians , the Orthopeadic teams and Trauma wards to ensure the best possible care is being provided for the patients.

Job responsibilities

Collate the relevant information about a patients pathway in line with the NHFD dataset criteria and ensure its accuracy before submission.

Complete the data entry of the relevant information and the day-to-day management of the databases (NHFD and FLSDB).

Manage database information securely, according to local and National Guidelines

Preparation of reports of key performance indicators for monthly governance meetings.

To assist with data queries and production of reports of patients who did not meet all the key performance indicators to discuss with the clinical lead and specialist nurse team.

Assist the Specialist Nurse teams to ensure that the patient best practice pathway is achieved wherever possible.

Liaise with the outpatients booking team regarding any follow up appointments for patients following their hip or fragility fracture following discharge.

Attend the monthly hip fracture M&M meeting to support the clinicians and assist in the accurate recording of the discussions.
