Job summary

The primary purpose of this role is to work dynamically across the PCN. The post holder will be responsible for co-ordinating our zero hours team of registered, un-registered and administrative staff, ensuring shifts are filled and the zero hours team are delivering a high standard of care, co-ordinating care and promoting good health to patients to reduce health and social inequalities, and supporting the management and delivery of PCN projects including the NHS Health Check programme and biannual covid and flu vaccination campaigns.

The role holder will also support the PCN lead for tackling health inequalities, working collaboratively with the Project and Business support Manager and practice staff to identify populations within the PCN who are experiencing inequality and developing plans to tackle the unmet needs of those populations. This will include promoting the uptake of Serious Mental Illness (SMI) annual physical health checks for patients registered with an SMI, the uptake of various cancer screening programmes, and undertaking activity to improve the coverage of blood pressure checks, by targeting need as informed by local data on health inequalities and potentially at-risk groups. The post holder will also assist in+ project planning and the monitoring of plans to ensure the project remains on target, highlighting and escalating any issues and risks proactively.

Job responsibilities

The role holder must have a positive can-do approach and be willing to work flexibly which includes working evenings and weekends during vaccination campaigns, as well as the ability to travel and work across various practice locations and outreach in the community.

There will be opportunities to develop project management skills within this role.

2. Key Duties and Responsibilities:

Covid vaccination clinics (twice yearly campaigns) - You will support the project delivery of and co-ordinate the clinics as well as the care of patients attending for their covid and flu vaccination. This will include:

- Work under the Operations and Performance Manager and, Project and Business Support Manager to support the delivery of the PCN covid vaccination and flu clinics

- Co-ordinate and support the zero hours vaccination team

- Contacting patients directly by telephone, text and email

- Support practices to co-ordinate care for patients living in care homes and who are housebound

- Organising community outreach clinics and roving teams

- Liaising with practices to ensure patient lists are uploaded and can be invited to appointments

- Actively working to identify barriers in engagement or update of vaccinations, developing solutions

- Monitoring and ordering of vaccines and consumables

- Staffing rotas and working with the PCN Senior Administrator to publish shifts

- Complete weekly returns and reports

NHS Health Checks You will co-ordinate PCN led NHS Health Check clinics to ensure a high level of patient care. Patients are eligible for this service if they are aged between 40-74 years of age, have no underlying health conditions, and have not received an NHS Health Check within the last 5 years. The key service outcome of the programme is to reduce the inequality in disability and premature mortality from cardiovascular disease through risk assessment, risk awareness and risk management.

This will include: -

- Communicating and work closely with practices to identify and invite patients to appointments, using population health data and searches to invite those most at risk

- Co-ordinating the clinics, including shifts and staffing

- Managing the review and processing of results, communicating with patients and co-ordinating follow up required by practices

- Support patients with onward follow up, referral and signposting

- Make every contact count and address personalised care needs of patients

- Coordinate outreach programmes and pop-up clinics, actively promoting NHS Health Checks

- Collect data on Key Performance Indicators and produce reports

Tackling health inequalities this role is key for assisting in how we tackle health inequalities. Your role will also be to work collaboratively with identified populations who have unmet needs and develop plans to address these.

This will include:

- Work with population health data

- Help develop plans to tackle unmet needs

- Promoting Learning Disability Health Checks

- Promoting the uptake of Serious Mental Illness (SMI) annual physical health checks for patients registered with an SMI

- Promoting the uptake of various cancer screening programmes

- Undertaking activity to improve the coverage of blood pressure checks and other cardiovascular prevention

- Championing health inequality work across the PCN

- Represent the PCN in various meetings and engagements

Project Management As the Project assistant, you will support the Project and Business Support Manager in the delivery of PCN projects. This will include:

- Supporting the writing of and delivery of project plans

- Support the writing of business cases and SBARs for proposed projects

- Support the writing of project evaluations

- Attending Network 4 board meetings and taking minutes

- Building relationships and fostering collaboration between practices

- Conduct research and write proposals as appropriate

Experience

Essential

Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work)

Desirable

Experience of working in a Primary Care setting Experience of working with technology and IT software in Primary Care EMIS Experience of collaborative working and building relationships across varied organisations

Qualifications

Good standard of education and high levels of numeracy and literacy (such as GCSEs grade A to C or equivalent)
