Job summary

Job Summary

To work under the supervision of the Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication to patients. To provide technical and administrative support to the Clinical Pharmacists and Clinicians. To support the practice team with queries around medication from patients; including face to face and telephone support to patients, primary and community staff.

Main duties of the job

KeyResponsibilities

Oversee theconversion of patients to Electronic Repeat Prescribing (ERD)

ProcessMedication Safety Alerts, communicating any such notifications to patients asrequired.

Regular liaisonwith community and acute hospital pharmacists.

Support theachievement of the practices prescribing Quality and Outcomes Frameworktargets.

Deal withmedication queries- acute medication requests, patient medication andprescription queries, with clinical support as required

Update andmaintain accurate patient medication records on the practice clinical computersystem, including advice given and action taken.

Assist the practicewith data analysis of prescribing behaviour in line with NICE guidance andlocal clinical commissioning directives.

Assist withthe development and review of medicine audits.

Provide a contactfor patients and colleagues with medication queries.

Projectwork/prescribing initiatives under the support of the clinical/prescribingstaff.

Link with hospital and community pharmacy colleagues to address discrepancies,supply issues and queries within competency.

Liaisewith the health care team including primary care staff, community nursesand care home staff with medication queries.

Perform regular audits using different tools

Document interventions made to patients medications in their health carerecords to ensure there is a clear audit trail and changes are clearlyaccounted for.

Seekadvice from the patients GP or practice lead GP as appropriate.

Essential

Related qualification in administration, or equivalent experience 1 years experience in a healthcare setting as a Pharmacy Technician or Prescription Clerk Excellent communication skills (written and oral) Clear, polite telephone manner Excellent IT skills including Microsoft Office packages and case management systems

Desirable

GPhC Registered Pharmacy Technician BTEC/NVQ level 3 or equivalent in pharmacy services Accredited Checking Technician Enrolled in, undertaking or qualified from, an approved training pathway.
