Job summary

Due to additional funding, we are able to offer a split post across the Southampton and East Hampshire Community Learning Disability Team (CLDT) and the Forensic Community Learning Disability Team (FCLDT) until 31 March 2025.

Within the FCLDT you will work within the MDT to assess the communication needs for people who have come into contact with the Criminal Justice System. We are a team and with a reduced caseload, allowing for a flexible and client-focused approach using the Good Lives Model of care. You will offer consultation to other professionals, as well as assessment and intervention to clients open to the team. This team is based in Totton covers all Hampshire.

Within the CLDT, you will support people to meet their communication and/or dysphagia goals. We work with adults with Learning Disabilities in the community, empowering them to live healthier and happier lives. This team has offices in Southampton and Fareham.

We will provide you with access to peer support, regular 1:1 meetings with the Lead Speech and Language therapist in both services, appraisals and clinical supervision. We actively encourage CPD, and attendance at relevant CENs. Additional support will be provided in the Forensic Team to support you in adapting your previous experience to the Forensic caseload therefore prior experience in this area is not essential.

Main duties of the job

To work as a member of the Community Learning Disability Team and the Forensic Community Learning Disability Team to provide Speech and Language Therapy assessment, treatment and management.

To manage a caseload of service users with communication and/or dysphagia needs as a result of complex mental health / learning disability needs, including evaluating referrals for intervention and priorities for treatment.

To work with service users to identify Speech and Language Therapy goals as part of the overall care plan; using specialist mental health / learning disability and Speech and Language therapy assessment tools.

To use a broad range of standardised and non-standardised assessments appropriate to the model of practice.

To plan and implement evidence based, person centred, individual and/or group interventions to achieve therapeutic goals.

To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of interventions.

To apply clinical reasoning skills to effectively manage clinical caseloads.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
