Main duties of the job

Fixed Term to cover maternity leave for 9 months

£23,603 fixed salarypoint Pro Rata Per Annum

You will work closely and inpartnership with the Primary Care Network clinical team, coordinating resourcesand information to support choice, access and delivery of personalisedcare.

Care Coordinators will beresponsible for consulting with patients and determining their needs,developing care plans, coordinating patient care services, education regardingtheir conditions and working with the medical team to evaluate interventions, holding group activities.

Consult with patients and family members to discuss their health and social problems including providing support, housing benefits. Support patients in their understanding and management of their condition and medications, within the remit of the role. Develop care plans to address personal health care needs.

Qualifications

Essential

QCF level 3 in Health and Social Care or equivalent Level 2 qualification in Numeracy and Literacy or equivalent level of competence.

Experience

Essential

Experience of working in a Multidisciplinary team. Experience of working with people in a healthcare or similar setting

Desirable

Experience of working with the client group. Experience of multi-disciplinary team working

Skills & Knowledge

Essential
