Job summary

We are excited to be advertising for a Registered Nurse for the Specialist Learning Disability Service with a focus on mental health and children with a Learning Disability.

The Service is a consist of Learning Disability Nurses. Clinical Psychologists , Occupational Therapists & Assistant Practitioners who offer a range of services for children and adolescents with a diagnosed Learning Disability and Autism from assessment and intervention to consultation, liaison and training for other agencies.

The service can support Children & Adolescents with a moderate to profound Learning Disability, in conjunction with complex behavioural , health or sleep issues related to this diagnosis.

Main duties of the job

Work within the Specialist Learning Disability Service, providing support and supervision to Learning Disability nurses working across the Specialist Learning Disability Service and all other integrated pathways

Ensure a strengths-based approach to assess, plan, implement and evaluate interventions to support complex packages of care for children with additional needs in the community

Contribute to risk assessment and care planning in collaboration with children, young people and their carers

Provide assessment, advice and specialist interventions to children, young people and their families and to hold a caseload of children with complex needs, leading on the mental health need's for CYP with a Learning Disability. Provide case management of an identified caseload of children and young people with highly complex long-term conditions and complex needs by utilising and promoting evidence-based care

To act as a source of expertise on their specialist clinical area

To advocate on behalf of CYP and to be at the forefront of clinical development within their specialist clinical area

To be able to effectively communicate with children, families and staff to be able to sensitively impart difficult news and respond appropriately to complaints and challenges

Excellent interpersonal and communication skills with the whole of CFHD

Represent occupational therapy and/or individual patient to ensure the delivery of a coordinated multi-disciplinary service and to integrate occupational therapy management in to the overall management plan. This may involve health/social care staff, voluntary and private sector. This will involve providing specialist advice to the multi-disciplinary team on management of recovery and discussion of patient care, progress and discharge planning

To communicate complex and sensitive condition-related information from assessment to children/young people and families, carers and members of the multidisciplinary team/other professions in a way that is accessible and easily understood. This will include facilitating strategies for people with communication disorders and learning disabilities where barriers to understanding may exist

To demonstrate empathy with children/young people families, carers and colleagues, ensuring that effective communication is achieved, particularly where barriers to communication exist, or sensitive issues are being dealt with. Children/young people and families and carers will frequently have difficulty accepting the information provided
