The shift pattern for this position is Mon-Fri.

This is a fixed term position of 1 year.

We are seeking a compassionate and self-motivated candidate for a Cancer Wellbeing Practitioner to join our Macmillan ICJ Team in East Dunbartonshire HSCP. This is an exciting post that will contribute to the delivery of the East Dunbartonshire HSCP Strategic Plan, and the Scottish Governments Cancer Strategy 2023.

The post holder will ensure the delivery of a defined work stream cross referencing to the Scottish Government and NHS Greater Glasgow & Clyde corporate themes and local health needs. Further providing expertise and advice to develop colleagues/partners and the communities capacity to promote health and reduce health inequalities, with a core focus on Cancer.

The role determines that, along with the ICJ Project Team, the post holder delivers a person-centred service as part of the East Dunbartonshire Macmillan ICJ Programme. The post holder is required to establish and maintain key partnerships with statutory, voluntary and community organisations. The position sits within a ‘Macmillan Professional’ branded role and duties include engaging, through a person centred approach, East Dunbartonshire residents who have received a cancer diagnosis. Key to the service delivery is the undertaking of a Holistic Needs Assessment, which leads onto the establishment of an individual care plan, which is owned by the service user.

Ideally, the post holder should have familiarity of: supporting people who have experienced the impact of a long-term health condition. The position requires the post holder to have open, empathetic, approach with effective and engaging communication skills along with the ability to manage challenging interactions in a respectful, supportive and non-judgmental manner.

The core functions of this post aim to progress public health priorities, including, supporting positive well-being outcomes for Long Term Conditions, enhancing delivery for social prescribing and wellbeing across Primary Care Services and utilising social media and other public communication domains to promote and reinforce key public health improvement campaign and messages

It is essential that the post holder has at least 3 years experience in a health or social care-based setting, have a driving license and access to your own car.

The role is offered as a 1 year fixed term appointment and offers a unique opportunity to work with a range of partners across health and social care, local authority, third sector and service users and their carers. This is fulltime role, working days are Mon-Fri (37hrs) with some home working is expected across the working week.

Ideally, educated to degree level but significant experience in a similar role will be strongly considered. You will have strong interpersonal skills and a belief in the strengths and resilience inherent in people and communities.
