Job summary

An exciting opportunity to support an innovative,collaborative and dynamic Primary Care Network, serving a population of over80K patients, one of the largest in North West London (NWL) ICS. You will workoperationally with the Senior PCN Development Manager, Digital andTransformation Leads and Clinical Directors to drive forward key patientinitiatives across our 7 practices within our local neighborhood team.

Developing excellent working relationships with key systemstakeholders, primary care leadership and practice managers across our PCN willbe pivotal to the role. Our PCN is well respected across Harrow Place BasedPartnership (PBP) and NWL ICS for providing good patient care and engaginglocal communities, with strong Patient Association co-production.

Established experiencing working within the NHS, healthcare,social or voluntary sector would be desirable, although other candidates may beconsidered, depending on merit and skill set.

Main duties of the job

We are looking to recruit to the post of Social PrescribingLink Worker, to work within our Primary Care Network.

Social prescribing enables all primary care staff and localagencies to refer people to a link worker. Link workers give people time andfocus on what matters to the person as identified through shared decisionmaking or personalised care and support planning. They connect people tocommunity groups and agencies for practical and emotional support. They workwithin multidisciplinary teams and collaborate with local partners to supportcommunity groups to be accessible and sustainable. Social prescribing complements other approaches such as activesignposting.

The successful candidate will play a key role in proactivelyidentifying and working with people, including the frail/elderly and those withlong-term conditions, to provide navigation of care and support across healthand care services.

They will work closely with GPs and practice teams,supporting them to understand and manage their condition and ensuring theirchanging needs are addressed. They will enable people to access the support requiredto meet their health & wellbeing needs, helping to improve peoples qualityof life.

The successful candidate will have excellent interpersonaland communication skills, and be organised, patient and empathetic. They willhave experience of working in health, social care or other support rolesincluding direct contact with people, families or carers.

Providing personalised support to individuals, their familiesand carers to take control of their wellbeing, live independently and improvetheir health outcomes;

Developing trusting relationships by giving people time andfocusing on what matters to them;

Taking a holistic approach, based on the personspriorities, and the wider determinants of health;

Co-producing and supporting delivery of a simplepersonalised care and support plan to improve health and wellbeing;

Introducing or reconnecting people to community groups andservices;

Evaluating the individual impact of a persons wellnessprogress;

Recording referrals within GP clinical systems using thenational SNOMED social prescribing codes;

Drawing on and increasing the strengths and capacities oflocal communities, enabling local VCSE organisations and community groups toreceive social prescribing referrals;
