Main duties of the job

Working as part of the practice multidisciplinaryteam, the post holder will ensure skilled nursing services are deliveredeffectively to the practice patient population. The practice nurse will beresponsible for delivering health promotion, chronic disease management, healthprevention, and well women clinics, as well as general treatment room duties such as wound care, administration of injections, childhood immunisations and ear irrigation.

About us

Stockwood Medical Centre is an ever-expanding practice with our practice patient population covering a variety of demographics. Situated on the southeast edge of Bristol we are in easy reach of both Bristol and Bath and close to the Somerset countryside. We provide care for approximately 10,600 patients and are active members of the Connexus Primary Care Network (PCN).

We are a friendly, dynamic and supportive team of 3 GP Partners, 5 salaried GPs, 2 ANPs, 1 Specialist Paramedic, 6 practice nurses, 3 HCAs & 2 phlebotomists, all supported by our experienced administrative and reception teams.

We regularly support student nurses for their community placement and welcome students from other courses to observe General Practice

We are also an established GP training practice and actively involved in both undergraduate and post graduate medical education. We offer minor surgery and joint injection clinics and are forward-thinking in how we develop staff and services to meet the needs of our patients.

Job responsibilities

MAIN DUTIES:

To assess patients health needs and implement nursing interventions to meet their needs. To manage the care and treatment of patients with stable or long term conditions within agreed protocols. To maintain high standards of infection control at all times, protecting patients, self and colleagues. To actively engage patients and carers in the management of their health, promoting health and wellbeing. To refer patients to other members of the practice clinical team in line with agreed procedures. To maintain accurate records of all clinical activity in line with practice protocols and NMC standards. To contribute to clinical governance activities within the team including audit and evaluation. To support other members of the practice team in maintaining health, safety and security, identifying and assessing potential risks and contributing to management. To use appraisal and supervision to identify development needs and maintain and develop competence in line with NMC requirements. To recognise patients readiness to change in supporting their attempts to change unhealthy behaviour To be competent in anaphylaxis and resuscitation techniques. To advise patients on the national and local cervical cytology and mammography screening programmes. To administer injections under an individualised prescription or Patient Group Directive (PGD). To provide an adult and childhood immunisation service. To ensure safe storage, rotation and disposal of vaccines. To perform wound care using aseptic techniques. To undertake opportunistic screening, including blood pressure checks, height/weight and BMI calculations, urinalysis and smoking status To ensure the highest standards of care are provided for all patients Person Specification

Relevant experience in Primary Care. Understanding of infection prevention and control measures in clinical practice. Experience of chronic disease management. Experience of working autonomously. Broad knowledge of clinical governance. Understands the requirement for PGDs and associated policy. Ability to follow clinical policy and procedure. Ability to contribute effectively to clinical risk management. Commitment to ongoing professional development. Effectively utilises resources. Disclosure Barring Service check. Maintains evidence of continuing professional development commensurate with the role and for ongoing Nurse revalidation.

Adult and Child Immunisation Accredited Qualification in Asthma / COPD / Spirometry / FENO Cervical Cytology (registered and updated within the last 3 years) Mentor or teaching qualification
