Main Responsibilities- Act as a key member of our nursing team, providing expert care to patients and supporting the delivery of high-quality patient care.
- Develop and implement effective care management programs for frail and housebound patients, ensuring their healthcare needs are met.
- Assess patient needs, providing evidence-based care and promoting health and wellbeing.
- Implement and evaluate individual treatment plans for chronic disease patients, promoting health and wellbeing.
- Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
- Provide routine nursing care to patients, adhering to clinical best practices and guidelines.
- Diagnose and manage acute and chronic conditions, prescribing medication as necessary.
- Coordinate the provision of travel medicine services, ensuring patients receive necessary vaccinations and advice.
- Maintain accurate clinical records, adhering to relevant legislation and guidelines.
- Ensure the effective use of chronic disease registers and participate in well woman and well man clinics.
- Support the team in dealing with clinical emergencies and recognize, assess, and refer patients presenting with mental health needs.
- Registered Nurse (Nursing and Midwifery Council) qualification.
- Experience of working autonomously and in a primary care environment.
- Knowledge of infection prevention and control measures, wound care, and ECGs.
- Ability to work under pressure and in stressful situations, prioritizing health issues and intervening appropriately.
- Commitment to ongoing professional development and a willingness to learn and adapt to new situations.
