Job Summary

Hillview Family Practice is seeking a skilled and dedicated Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our practice.

Main Responsibilities
  • Act as a key member of our nursing team, providing expert care to patients and supporting the practice's quality improvement initiatives.
  • Develop and implement effective care management programs for frail and housebound patients, ensuring their needs are met and their health is optimized.
  • Assess patient needs, providing evidence-based care and promoting health and wellbeing through health promotion and wellbeing programs.
  • Implement and evaluate individual treatment plans for chronic disease patients, ensuring they receive the best possible care.
  • Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Provide routine nursing care to patients, adhering to clinical guidelines and best practices.
  • Diagnose and manage acute and chronic conditions, prescribing medication as necessary.
  • Coordinate the provision of travel medicine services and oversee the provision of chronic disease clinics.
  • Maintain accurate clinical records, ensuring compliance with legislation and best practices.

Requirements
  • Registered Nurse (Nursing and Midwifery Council) qualification.
  • Experience of working autonomously and as part of a team.
  • Knowledge of infection prevention and control measures.
  • Ability to work under pressure and prioritize tasks effectively.
  • Commitment to ongoing professional development and evidence of continuing professional development (CPD).
