Job summary

We are looking for a motivated and caring experiencedPractice Nurse to join our friendly and supportive Practice team. We are aprogressive and forward-thinking Practice who are passionate about providinghigh quality patient care.

Main duties of the job

Provide Assessment, Screening, Treatment Services and Health Education advice Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols Provide general and specific health screening to the practice patients (within agreed protocols) with referral to the general practitioners as necessary Participate in health promotion for patients with chronic diseases such as diabetes, asthma and CHD, according to agreed practice protocols Manage leg clinic and facilitate leg management pathway. Prepare/assist/facilitate vaccinations and immunisations and travel programmes Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc., and perform any investigatory procedures requested by the GPs following appropriate training Take the lead in an area of chronic disease management Ensure accurate notes of all consultations and treatments are recorded in the patients computerised medical records Co-operate and participate in the collection of statistics for audit projects and evaluate and present results as required Have the opportunity to participate in research with the practice and wider PCN. The research role will give you the opportunity to work alongside our research team, who are supported by the University of Bristol. Have the opportunity to be involved with our Covid medicine delivery unit providing infusions for our vulnerable population requiring treatment.

MAIN TASKS

Clinical Responsibilities

Workingas part of the practice nursing team

Provide Assessment, Screening, Treatment Servicesand Health Education advice

Providenursing treatments to patients in participation with general practitioners orindependently to agreed protocols

Providegeneral and specific health screening to the practice patients (within agreedprotocols) with referral to the general practitioners as necessary

Giveappropriate health promotion advice to patients in whom risk factors areidentified, according to agreed practice protocols and local and nationaltargets

Advisepatients on general health care and minor ailments with referral to generalpractitioners as necessary

Participatein health promotion for patients with chronic diseases such as diabetes, asthmaand CHD, according to agreed practice protocols.

Manageleg clinic and facilitate leg management pathway.

Prepare/assist/facilitatevaccinations and immunisations and travel programmes

Undertakecytology screening or be willing to learn.

Undertakethe collection of pathological specimens including intravenous blood samples,swabs, etc., and perform any investigatory procedures requested by the GPsfollowing appropriate training

Provideappropriate regular follow up for patients when necessary

Takethe lead in an area of chronic disease management

Administrativeand Professional Responsibilities

Participatein the administrative and professional responsibilities of the practice team

Ensureaccurate notes of all consultations and treatments are recorded in the patientscomputerised medical records

Ensureaccurate completion of necessary documentation associated with patient healthcare and registration with the practice

Attendand participate in all practice meetings as required

Assistin the formulation of practice philosophy, strategy and policy

Undertakeall administrative procedures relating to the post, liaising with the NurseTeam Lead

Co-operateand participate in the collection of statistics for audit projects and evaluateand present results as required

Participatein the review of existing evidence-based nursing practice by using valid andreliable audit tools and published and validated research results from medical,nursing and paramedical data to effect changes in clinical practice

Coverin the absence of other treatment room staff as and when necessary

ResearchProjects

Havethe opportunity to participate in research with the practice and wider PCN. Theresearch role will give you the opportunity to work alongside our researchteam, who are supported by the University of Bristol.

Infusion Clinics

Havethe opportunity to be involved with our Covid medicine delivery unit providinginfusions for our vulnerable population requiring treatment.

Educationand Training of Students and Practice Staff

Participatein the induction of all members of practice staff. To be available for students of alldisciplines to observe clinics, answer questions arising from these clinics andteaching new skills where necessary

Liaison

Maintaineffective liaison with other agencies and staff concerned with patient care andwith all other disciplines

ProfessionalDevelopment

Maintaincontinued education by attendance at course and study days as deemed useful ornecessary for professional development and to maintain PREP requirements

SPECIAL REQUIREMENTS FOR THE POST

RegisteredGeneral Nurse

Membershipof a professional body

Towork in accordance with the UKCC Code of Conduct

Excellentcommunication skills

Change-management skills and ability to support patients to change lifestyle. Good Communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills. IT skills - EMIS Web clinical system experience. Management of long-term conditions. Involvement in implementing and using protocols and clinical guidelines. Experience of audit.
