An exciting opportunity has arisen for a dynamic, motivated and innovative pharmacist to join our expanding Medicines Management team at Central London Community Healthcare Trust.

Main duties of the job

We are looking for a pharmacist to provide a clinical pharmacy service at our bedded units within Hertfordshire & North London (Full- time, Part- time, Flexible working will be considered)

As an integral member of the team, you will be providing specialist expertise in the use of medicines whilst supporting to address both public health and social care needs of patients. You will be involved in providing and developing a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team supported by the Lead Pharmacist for Hertfordshire and Lead Pharmacist for Bedded Services.

If you are successful, you will join the existing team of Band 7 pharmacists at the Trust covering a range of rehabilitation specialties. You will be supported by experienced Senior Clinical Pharmacists allowing you to gain new skills in each rotation and the ideal opportunity to develop strategic, quality improvement and leadership skills.

The post holder may be required to travel across the different sites within the Trust where we have Bedded Services Inpatients, therefore should hold a full, valid, UK driving licence and have access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010)

Job responsibilities

A detailed Job Description and main responsibilities can be found in the attached JD. Some of the duties include the following:

To provide and develop a high-quality specialist Clinical Pharmacy Service across a range of inpatient settings in the Community Hospitals (Rehabilitation Units, Interim Care Beds, Palliative Care, Discharge to Assess ward) covered by the Bedded Services team supported by the Lead Pharmacist for Hertfordshire and Lead Pharmacist for Bedded Services. To provide support and guidance to Medicine Management Pharmacy Technicians To be responsible for individualised patient pharmaceutical care ensuring clinical prioritisation on a day-to-day basis at ward level. To provide a patient focused pharmacy service to patients and staff including medicines reconciliation and full clinical screening of prescriptions to ensure safe and cost-effective use of evidence based and rational use of medicines, in line with local policies and national guidance ensuring medicines are optimised and risks to patients are minimized NICE, NHSE/I, PHE Facilitate changes to the patients own use of medicines to improve safety, adherence and empower as much independent use of medicines as possible and appropriate. To support and implement efficient medicines management initiatives to reduce drug waste and drug spend and to support patient flow and facilitate timely discharges To participate in ward rounds and MDTs where appropriate to ensure a multidisciplinary and integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings.

Person Specification

Education/Qualifications

Essential

Vocational Masters Degree in Pharmacy. Professional registration with the General Pharmaceutical Council (GPhC). Postgraduate Clinical Pharmacy Certificate qualification or equivalent

Independent prescriber.

Experience

Hospital experience Experience of providing a Clinical Pharmacy Service with good medicines management initiatives Evidence of basic teaching and tutoring in a hospital setting Experience of working effectively within multi-disciplinary groups Experience of structured medication review

Sklils and Knowledge

Essential

Understanding of the NHS and political environment Sound knowledge of prescribing, pharmacy and medicines management issues Sound knowledge of Medicines Reconciliation on admission and discharge and Medicines Management processes Good core therapeutic knowledge Excellent communicator - both verbal and written Able to manage own time and prioritise workload effectively Computer literate - working knowledge of office applications, internet, email and electronic diary systems and advanced spreadsheet skills Adaptability and flexibility in approach to meet service needs Ability to think clearly and work under pressure l organized Self-motivated with an enthusiastic approach to work
