Senior Medicines Optimisation Pharmacist

Motivation, forward-thinking and a drive to improve patient care are essential qualities for this pharmacist role in the NHS. As part of the Mottingham, Downham & Chislehurst Primary Care Network team, you will be responsible for optimising medicines and providing expert advice on clinical matters.

Main Responsibilities
  • Develop and implement medicines management processes and policies to reduce medicine-related risks and improve patient adherence.
  • Conduct structured clinical reviews with patients to manage complex polypharmacy needs and provide professional support on safe prescribing, handling and administration of medicines.
  • Work collaboratively within a Multi-Disciplinary Team (MDT) to clinically assess and treat patients, resolving day-to-day medicines and prescription issues with healthcare providers.
  • Lead on the medicines aspects relating to patient care, presenting changes to prescribing in line with NICE guidelines and promoting safe, cost-effective rational prescribing.
Requirements
  • Registered with the General Pharmaceutical Council.
  • Postgraduate qualification in clinical/community pharmacy or equivalent additional experience.
  • Evidence of recent continuing learning/professional development.
