We are looking for a motivated and enthusiastic AdvancedNurse Practitioner to join our friendly team working in the town of New Millsin the beautiful High Peak, close to Stockport.We are a small practice with around 4600 patients, and we value theability which this list size gives us to get to know our patients well asindividuals. Our shared ethos allows usto practice a more traditional style of patient-focused family medicine with agood level of continuity of care. We arelooking for someone who will enjoy working as part of our multidisciplinaryteam and is also confident to work independently within their professionalcapabilities. We believe that this rolewould best suit someone who is already experienced in working independently inGeneral Practice. We are looking forsomeone to work part time, between 16 and 24 hours/week.

Main duties of the job

The successful candidate will work alongside our multi-disciplinaryteam of GPs, practice nurse, HCAs and clinical pharmacist to provide care andtreatment to patients with either long term conditions or undiagnosedundifferentiated diagnoses. You willwork as an autonomous practitioner, diagnosing, managing, prescribing anddischarging patients with illness or injury across all age groups. You will also support and lead the practicenursing team.

A key part of the role will be taking on the role ofCommunity Matron for our housebound frail and vulnerable patients. In this role you will work closely with themulti-disciplinary team and practice care co-ordinator to identify thosepatients most at risk and work pre-emptively to monitor their chronicconditions and identify their clinical needs to reduce the need foradmission.

You will also lead on the care and monitoring of ourpatients in two residential homes for adults with learning disabilities, withthe support of the GPs.

We also envisage a part of this role will include seeingpatients in an acute clinic setting in the practice.

About us

We are an established practice with excellent team working,and our practice area covers New Mills, Hayfield, and the surrounding area,extending part way into Disley at one end and Whaley Bridge at the other. We use EMIS as our clinical system. We arefortunate to work from a large purpose-built premises in the centre of NewMills.

We are part of High Peak and Buxton PCN, made up of 8 practices from New Mills toHartington. We are active members ofthis PCN and enjoy good working relationships with all our neighbouringpractices

Our clinical team comprises 2 GP partners, 1salaried GP, 1 practice nurse, 2 HCAs and 1 clinical pharmacist andsupported by the widercommunity and PCN multi-disciplinary team, the PCN pharmacy hub forgeneral medication queries, First Contact Practitioners, Mental Health Workerand Social Prescribers. The PCN homevisiting service is able to undertake most of our acute home visitrequests. We have an excellent Practice CareCo-ordinator who chairs our weekly MDT meetings and ourexcellent District Nursing team with whom we have a close and collaborativeworking relationship.

We also have a weekly clinical meeting so allclinicians can informally discuss patients of concern and discuss any newdevelopments within the practice.

We are a training practice who welcome GP specialisttrainees as well as medical students.

We close once a month on a Wednesday afternoon for full teamtraining. Some of these sessions arejoint with our PCN.

Responsibilities and duties will include, but may not belimited to the following

To provide assessment, diagnosis and treatment,of both acute and chronic conditions, acting as an autonomous clinician withthe support and guidance from the GPs. This will include initiating appropriatetreatment and /or referral, including admission to hospital if required. It would be desirable to have the skills toassess children as well as adults.

To act as practice liaison to our tworesidential care homes for adults with learning disabilities as well as arrangingregular weekly visits to assess both acute and chronic conditions and undertakeannual LD reviews.

To proactively manage our housebound and frailpatients with input from our practice care co-ordinator, to identify any earlyclinical need and reduce deterioration and admission. This will include visiting them at home andassessing any acute or chronic conditions.This may include working with outside agencies to ensure appropriatecare.

To undertake a share of daily home visits ifneeded.

To maintain adherence and familiarity withlocally agreed healthcare pathways and signpost appropriately to otherservices.

Ability to support GP administrative workloadwith suitable training and support

Support and lead the practice nursing teamincluding mentoring as appropriate.

To proficiently use the clinical IT system forsafe and thorough documentation of patient care as well as extract informationfor clinical governance.

To work collaboratively with other members ofthe health care team such as the District Nursing Team, Mental Health Services,staff in Care Homes, Ambulance Service, and Secondary Care, etc.

To engage in self-appraisal and ensure practiceis within good clinical governance and based on sound up-to- date clinicalevidence. Where appropriate, to identify and assess any risks associated withpatient care or to oneself and communicate these risks to the appropriateclinical manager.

To ensure effective time management andprioritisation of workload.

Ability toworkwithin own scope of practice and understanding when to refer to GPs

To participate and contribute to clinicalmeetings.

To work within the requirements of DataProtection legislation as it applies to patient records.

To maintain current professional registrationand indemnity.

Qualifications

Essential

Active Registration with NMC and meet NMC revalidation requirements. Recognised ANP qualification at minimum of degree level or equivalent. Independent prescriber. Experience of advanced clinical practice with a high degree of autonomy and complex decision-making. Minor illness. Chronic disease management. Full UK Driving License and use of a car to undertake home visits if required.
