Job summary

Role Purpose : Integral part of the clinical team. Care for patients with acute care needs and provide continuity of care when appropriate. Responsibilities : Act within professional boundaries. Provide first-line clinical assessment via telephone or online triage. Conduct face-to-face assessments when necessary and best practice. Take patient history, perform clinical assessments, provide diagnosis, treatment, and evaluate care. Clinical Decision-Making : Demonstrate safe, expert clinical decision-making. Work within the Oak Hall clinical team. Seek support or escalate cases when outside of scope. Collaborate with GPs for shared patient care.

Main duties of the job

Support the surgery team in providing high-quality care and enhancing patient experience Contribute to service improvement and workforce satisfaction

Training and Development:

Involve in training and developing the wider clinical and non-clinical team

Collaboration:

Work collaboratively within the multidisciplinary general practice team Support the delivery of policy, procedures, and provide nurse leadership as needed

Candidate Requirements:

Highly motivated ANPs and NPs with independent prescribing skills

Responsibilities:

Provide timely, high-quality, person-centred clinical care Follow up with patients, manage referrals, and action results of investigations and medication requests

We are a high-achieving GMS Practice with strong QoF results, proudly serving a patient population of over 5,800. Our dedicated team consists of 2 GPs, 3 Nurse Practitioners, skilled Nurses, HCAs, Phlebotomist, Pharmacists, and a knowledgeable and long-serving Administrative and Reception team. We utilise EMIS Web, Docman, and Ardens Manager to ensure streamlined and effective patient care.

Job responsibilities

Telephone Triage : Triages patients via online forms or telephone conversations and assesses the most appropriate care setting. Assessment and Treatment : Assesses, diagnoses, plans, implements, and evaluates treatment for minor illnesses, undifferentiated diagnoses, or complex needs. Emergency Care : Prioritises health problems and provides interventions in urgent or emergency situations. Record Keeping : Maintains accurate and complete patient records in line with legislation and policies. Quality Governance : Participates in quality governance and uses the audit cycle to implement improvements. Nursing Services : Assists in organising and coordinating nursing services to provide evidence-based practice. Acute Care : Works with the Duty Team to manage and care for patients with acute or urgent needs. Long-Term Conditions : Identifies, diagnoses, and formulates treatment plans for patients at risk of long-term conditions. Condition Management : Works with healthcare professionals to manage long-term conditions using evidence-based methods. Medication and Follow-Up : Prescribes safe and effective medication, reviews adherence, and follows up based on healthcare needs. Health Promotion : Supports patients in adopting health promotion strategies and self-care principles. Vaccination : Implements and participates in vaccination programmes, including travel vaccinations. Women's Health : Manages women's health needs, including family planning and cervical cytology. Learning Environment : Promotes a learning environment, participates in training, and mentors learners. Cover and Support : Provides cover for colleagues and offers support and mentoring for Health Care Assistants. Meetings : Participates in or leads practice meetings and clinical team meetings. Infection Control : Applies infection control measures according to guidelines. Independent Prescribing : Independently prescribes to patients following protocols. Diagnostic and Referral : Books diagnostic investigations, refers patients to other services, and interprets pathology reports. Community Health Promotion : Works with the local community to provide health promotion information. Supervision and Training : Supervises or supports the training of junior clinical team members. Qualifications

Essential

RGN Qualification: Must hold a valid RGN qualification Independent Nurse Prescriber: Must be a qualified Independent Nurse Prescriber registered with the NMC NMC Revalidation: Must meet NMC revalidation requirements as outlined in the NMC Revalidation booklet

Desirable

Qualifications: For qualifications obtained after December 2020: A Master s degree is required with reference to RCN Credentialing for Advanced Level Nursing Practice For qualifications obtained up to December 2020: A Postgraduate diploma or degree in Advanced Practice is required Accredited Training in the following areas: Minor Illness Urgent Care Chronic Disease Management or Frailty
