Job SummaryWe are seeking a highly motivated Advanced Nurse Practitioner to join our clinical team. Key Responsibilities- Telephone Triage: Assess and manage patient calls, providing timely and effective interventions to ensure the best possible outcomes.
- Assessment and Treatment: Conduct thorough assessments, diagnose, and develop treatment plans for patients with minor illnesses, undifferentiated diagnoses, or complex needs.
- Emergency Care: Prioritize health problems and provide interventions in urgent or emergency situations, ensuring the safety and well-being of patients.
- Record Keeping: Maintain accurate and complete patient records, adhering to legislation and policies.
- Quality Governance: Participate in quality governance and contribute to the implementation of improvements, ensuring the highest standards of care.
- Nursing Services: Assist in organizing and coordinating nursing services to provide evidence-based practice.
- Acute Care: Work with the Duty Team to manage and care for patients with acute or urgent needs.
- Long-Term Conditions: Identify, diagnose, and formulate treatment plans for patients at risk of long-term conditions.
- Condition Management: Collaborate with healthcare professionals to manage long-term conditions using evidence-based methods.
- Medication and Follow-Up: Prescribe safe and effective medication, review adherence, and follow up based on healthcare needs.
- Health Promotion: Support patients in adopting health promotion strategies and self-care principles.
- Vaccination: Implement and participate in vaccination programs, including travel vaccinations.
- Women's Health: Manage women's health needs, including family planning and cervical cytology.
- Learning Environment: Promote a learning environment, participate in training, and mentor learners.
- Cover and Support: Provide cover for colleagues and offer support and mentoring for Health Care Assistants.
- Meetings: Participate in or lead practice meetings and clinical team meetings.
- Infection Control: Apply infection control measures according to guidelines.
- Independent Prescribing: Independently prescribe to patients following protocols.
- Diagnostic and Referral: Book diagnostic investigations, refer patients to other services, and interpret pathology reports.
- Community Health Promotion: Work with the local community to provide health promotion information.
- Supervision and Training: Supervise or support the training of junior clinical team members.
- Qualifications: Essential qualifications include a valid RGN qualification, Independent Nurse Prescriber registration with the NMC, and NMC revalidation requirements.
- Desirable Qualifications: A Master's degree in Advanced Practice or a Postgraduate diploma in Advanced Practice, with reference to RCN Credentialing for Advanced Level Nursing Practice.
- Experience: Essential experience includes autonomous practice in an ANP or NP role, primary care experience, and triage expertise. Desirable experience includes audit experience, QOF data management, and clinical supervision.
