Job Title: Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at Oak Hall Surgery. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to our patients.

Key Responsibilities:
  • Provide comprehensive assessments, diagnoses, and treatment plans for patients with acute and chronic conditions.
  • Work collaboratively with the multidisciplinary team to ensure seamless care coordination and patient outcomes.
  • Develop and implement evidence-based practice guidelines to improve patient care and outcomes.
  • Participate in quality governance and audit activities to ensure high-quality care.
  • Supervise and mentor junior clinical team members to ensure continuity of care.
Requirements:
  • Valid RGN qualification and Independent Nurse Prescriber registration.
  • Proven experience working independently in an ANP or NP role.
  • Primary care experience and triage expertise.
  • Ability to work collaboratively as part of a multidisciplinary team.
