Job Title: Advanced Nurse Practitioner

We are seeking an experienced Advanced Nurse Practitioner to join our team at Oak Hall Surgery. As an Advanced Nurse Practitioner, you will play a key role in providing high-quality patient care and contributing to the development of our primary care services.

Key Responsibilities:
  • Provide comprehensive assessments and diagnoses for patients with acute and chronic conditions.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Prescribe medications and treatments as necessary, in accordance with our clinical guidelines.
  • Collaborate with our multidisciplinary team to ensure seamless care coordination and communication.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Postgraduate diploma or degree in Advanced Practice or a related field.
  • Experience working in primary care or a similar setting.
  • Strong assessment and diagnostic skills, with the ability to think critically and make sound judgments.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
