Main duties of the job

The successful applicant will join our friendly team of rotational pharmacists. We provide clinical rotations through medicine (including cardiology, neurology, respiratory, endocrinology and gastroenterology), surgery, acute admissions, paediatrics and aseptic services.

Although this post is based at Musgrove Park Hospital, as Somerset NHS Foundation Trust we provide clinical pharmacy services to two acute hospitals, specialist mental health units and community hospitals across Somerset, so there is opportunity to gain experience in a wide variety of settings and to work alongside pharmacists specialising in these areas.

You will be supported by our team of senior pharmacists (including independent prescribers) and by accredited medicines optimisation technicians and assistants as we expand our ward based Medicines Optimisation Teams.

Your training opportunities will be further supported through the opportunity to undertake a fully funded Clinical Pharmacy Diploma (including Independent Prescribing qualification).

Job Purpose:

To act as a clinical pharmacist within the Trust providing high quality and professional input into the delivery of clinical and operational pharmacy services

To work in all sections of the pharmacy service on a rotational basis under the direction of the relevant lead pharmacy staff. These areas include dispensary, medicines information, medical and surgical wards, and chemotherapy production.

Support safe, effective and efficient medicines and medicines information provision for patients and/ or carers, medical and nursing staff and other healthcare professionals throughout the whole organisation. This will include directing and checking the dispensing or issuing of medicines from all areas of the pharmacy department and screening for legality, drug incompatibilities, interactions and prescribing errors.

To provide high quality clinical pharmacy services at ward level as a part of ward based Medicines Optimisation Teams meeting the needs of patients and healthcare professionals within the trust. This includes medicines reconciliation, the screening of discharge summaries and oversight of ward based dispensing.

Support clinical governance processes, by incident reporting, following Trust and departmental guidelines, policies and procedures.

Support the delivery and development of new clinical pharmacy services to facilitate pharmaceutical care.

To actively participate in mandatory Continuing Personal Development (CPD) Programme as required by the General Pharmaceutical Council (GPhC)

Participate in continuing education through appropriate training schemes, clinical diploma and independent prescribing where appropriate, and self-directed learning, in line with clinical governance requirements.

To participate in the on-call, evening, weekend and bank holiday rotas.

Duties and Responsibilities

Communication and Key Working Relationships

This post will involve communicating with a variety of individuals via a variety of media including face to face, MS Teams, telephone, letters, electronic communications and high quality reports.

Planning and Organisation

*Prioritising own workload.

*When the sole pharmacist in dispensary, ensuring work is coordinated and prioritised in conjunction with the senior technicians.

*Take a ward based clinical pharmacy workload, visiting allocated wards and seeing each patient on those wards daily complying with all departmental clinical pharmacy standards.

*Work as part of the Medicines Optimisation Team to identify patients at ward level who need medicines optimisation prioritisation and input.

*To provide cytotoxic manufacturing services to patients of the trust. This includes dispensing and set-up, technical screening of paperwork for manufacture of cytotoxic drugs and performing the final check and release of manufactured cytotoxic drugs.

Analytics

*Provide safe and efficient service to inpatients, day patients and outpatients.

*Analyse drug charts and patient information in order to provide advice on medicines and dosage.

*Analyse discharge summaries for accuracy and quality of information in relation to medication.

*Analyse patient's blood results in relation to their drugs and provide advice to medical staff.

Responsibility for Patient / Client Care, Treatment & Therapy

*Demonstrate a whole-system, patient-focused approach.

*Provide high quality specialist clinical pharmacy services on a daily basis.

*Act as an exemplar in the provision of clinical pharmacy services and medicines management by providing expert clinical pharmacy advice to clinicians, pharmacy staff and other healthcare providers on related drug prescribing and medicines management issues.

*To assist in medicines governance in the clinical setting, ensuring risks are managed and assurance of best practice achieved for specifically identified risks.

*To provide educational / instructional support to medical and nursing staff, other healthcare professionals and patients.

*To participate in general clinical pharmacy ward cover and dispensary services to facilitate continuous pharmacy services.

*To work closely with, and supervise the work of any Medicines Management Technicians and Assistants working within the Medicines Optimisation Teams.

*Undertake data collection to evaluate the effectiveness of the role of clinical pharmacy in the Trust including intervention monitoring, audits etc.

*To undertake and participate in research projects as agreed with the clinical pharmacy manager.

*Appraise the applicability of research evidence, and incorporate into clinical practice where appropriate.

*Provide clinical pharmacy training for Trainee Pharmacists, Pharmacy Technicians and Students as defined by national training standards as part of the structured training package ( lectures, accompanied ward visits, competency assessments and tutorials).

*Contribute to the appraisal, objective setting and personal development plans of clinical pharmacy staff.

*To supervise the activities of junior pharmacists undertaking clinical pharmacy activities and prioritise ward cover in their absence.
