Job overview

This post is a fantastic opportunity to work in the Early Intervention Services in Psychosis for Derby City and South supporting patients between the ages of - years of age with their physical health needs.

The post is a month fix term, part - time contract for a band 4 nursing associate who would be interested supporting patients with their physical health needs in the community, whilst working as part of a dynamic team supporting patients with a first episode of psychosis.

The post holder will be supported by both the clinical lead and manager of the team and will receive regular supervision and support within you role, including appraisals.

The post holder will be frequently working autonomously and will not be working under direct observation. Additionally, the post holder will occasionally have to make decisions regarding patient care and will need to feed back to professionals in the community.

The post holder will be responsible for reporting observations to registered professional staff together with raising incidents or areas of concern.

The post holder will be expected to sign post patients to other services if required along with ensuring up to date documentation is completed in a timely manner.

Main duties of the job

The post holder will be expected to organise their own clinics and to be able to complete patients checks along with requiring to attend home visits if necessary.

The post holder will be committed to developing and maintaining good working relationships with both primary care team and secondary care teams across the area.

The post holder will be expected to engage in the audit process for the trust and EIS and be able to have good IT skills to record appropriate records in a timely manner.

Motivated to maintaining high standards in a changing service

To perform blood tests, basic observations, smoking cessation and health promotion

Ability to assess capacity to consent and take appropriate action where required

To maintain high levels of infection control

This role requires working across the Derby City/ South Dales and South Derbyshire, this includes Erewash and Amber Valley
