Job summary

We are looking for a committed and compassionate Pharmacy Technichan to join our multi-disciplinary primary care team.

Main duties of the job

To be part of theHayling Island and Emsworth Primary Care Network (PCN), working as a key memberwithin the PCN Pharmacy Team, contributing to prescribing of the highestquality, safety and value and supporting medicines optimisation.

The post holderwill provide support to the GPs, Pharmacists, other clinical staff andreception teams to co-ordinate discharge summaries, medication reviews andensure the safe prescribing of medication. The post holder will support thedevelopment of a safe, cost effective, efficient practice repeat prescribingsystem through reviewing, improving and implementing medicines management processes.

Job description

Job responsibilities

The PharmacyTechnician role includes and is not limited to:

Support the Clinical Pharmacists andGPs with the review of hospital-initiated medication changes in accordance withpractice protocols and update patients medical records accordingly.

Support the staffinvolved in the preparation of repeat prescriptions and participate in thedaily preparation of repeat prescriptions to ensure safe and timely repeat prescribing.

Respond to medication queries frompatients, staff, community pharmacies and secondary care.

Contribute to thereview and development of repeat prescribing protocols / Standard OperatingProcedures to improve the efficiency and effectiveness of repeat prescribing systems.

Prepareevidence-based resources and information to support prescribing.

Promote the utilisation ofElectronic Prescription (EPS) and Electronic Repeat Dispensing.

Participate inand contribute to audits including significant event audits.

Take a proactive approach to thesafe handling of prescriptions, including assessment of risk and makingrecommendations for improvement.

Implement servicechanges relating to prescribing in accordance with local guidelines,legislation, safety updates NICE, MHRA.

Highlightmedication errors / queries and report any concerns to GPs / Pharmacist tosupport local clinical governance structures and working, for the correctmonitoring of high risk drugs.

Provide trainingfor practice staff on repeat prescribing systems.

Comply withconfidentiality and data protection policies.

Apply PCN andindividual practices policies, standards andguidance.

Assist in theinduction of medical students / registrars.

Establish effective workingrelationships within the Pharmacy Team and practice staff.

Undertake otherduties as requested by the Partners or LineManager.

Training and personaldevelopment

The post-holderwill continue to develop their skills, knowledge and undertake learning asappropriate. The Post-holder will participate in any training programmeimplemented by the Practice as part of this employment, to include:

Participation inin-year and annual appraisal.

Training needs identified as part ofany appraisal, system improvement, quality improvement.

Mandatory training.

Participation inPCN, whole practice training.

Communication

The post-holderwill:

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding andensure patients are fully informed and consent to treatment.

Effectivelycommunicate with members of the practice teams and external agencies to meetpatient need.

Other Duties:

Help cover staffholidays and sickness when required.

Participate in practice and team meetings as required.

Observe andcomply with all appropriate statutory requirements in relation to Child andAdult Safeguarding and to be familiar with the process for reporting concerns.

Keep up to datewith and carry out duties in line with the IG Compliance Toolkit.

In the course of seeking treatment, patients entrustus with, or allow us to gather, sensitive information in relation to theirhealth and other matters. They do so in confidence and have the right to expectthat staff will respect their privacy and act appropriately.

In the performance of the duties outlined in thisJob Description, the post-holder may have access to confidential informationrelating to patients and their carers, practice staff and other healthcareworkers. They may also have access to information relating to the PCN as abusiness organisation. All such information from any source is to be regardedas strictly confidential.

Information relating to patients, carers,colleagues, other healthcare workers or the business of the PCN or individualpractices may only be divulged to authorised persons in accordance with PCN andindividual practice policies and procedures relating to confidentiality and theprotection of personal and sensitive data.

Health & Safety:

The post-holderwill assist in promoting and maintaining their own and others health, safetyand security as defined in Health & Safety Policies in each individualpractice, to include:

Using personal security systemswithin the workplace according to individual practices guidelines.

Taking all reasonable personalprecautions necessary to avoid any potential health risks to colleagues, peoplereceiving care and the public.

Identifying risks in work activitiesand undertaking such activities in a way that manages those risks.

Making effective use of training to update knowledgeand skills.

Applying appropriate infectioncontrol procedures and measures within each practice according to local andnational guidelines and in accordance with practice policy.

Maintaining workareas and practices to ensure they are safe and free from hazards, and conformto health, safety and security legislation, policies, procedures and guidelines.

Reporting potential risks identified.

Candidates may also be required to work from our Emsworth site.

Person Specification

Qualifications

Essential

You'll need to complete a two-year accredited pharmacy technician course involving a mix of practical work experience and study. Level 3 NVQ Diploma in Pharmacy Service Skills and the Level 3 Diploma in Pharmaceutical Science

Desirable

Ideally candidates would have completed their CPPE Pathway.
