Position Overview

We are seeking a dedicated and empathetic Medication Management Specialist to become a vital part of our collaborative primary care team.

Key Responsibilities

As a member of the Hayling Island and Emsworth Primary Care Network (PCN), you will play an essential role within the PCN Pharmacy Team, ensuring the delivery of high-quality, safe, and effective prescribing while enhancing medication management.

Your responsibilities will include:

  • Assisting Clinical Pharmacists and General Practitioners in reviewing medication adjustments initiated by hospitals, in line with established protocols, and updating patient records accordingly.
  • Supporting the preparation of repeat prescriptions and participating in daily processes to guarantee safe and timely medication refills.
  • Addressing medication inquiries from patients, healthcare staff, community pharmacies, and secondary care providers.
  • Contributing to the development and refinement of repeat prescribing protocols and Standard Operating Procedures to enhance the efficiency and effectiveness of the prescribing system.
  • Creating evidence-based resources and information to support prescribing practices.
  • Encouraging the use of Electronic Prescription Services (EPS) and Electronic Repeat Dispensing.
  • Engaging in audits, including significant event audits, to ensure quality and safety.
  • Proactively managing the safe handling of prescriptions, assessing risks, and recommending improvements.
  • Implementing service modifications related to prescribing in accordance with local guidelines and safety updates.
  • Reporting medication errors or concerns to the appropriate healthcare professionals to support clinical governance.
  • Providing training to practice staff on repeat prescribing systems.
  • Adhering to confidentiality and data protection policies.
  • Applying the policies, standards, and guidance of the PCN and individual practices.
  • Assisting in the orientation of medical students and registrars.
  • Fostering effective working relationships within the Pharmacy Team and with practice staff.
  • Performing additional duties as assigned by management.

Professional Development

The successful candidate will be committed to ongoing professional growth, participating in training programs as part of their employment, including:

  • Annual appraisals and in-year reviews.
  • Identifying training needs through appraisals and quality improvement initiatives.
  • Mandatory training sessions.
  • Participation in PCN-wide training activities.

Communication Skills

The role requires effective communication with patients and caregivers, ensuring that alternative communication methods are utilized to accommodate varying levels of understanding, thereby ensuring informed consent for treatment.

Additionally, you will need to communicate effectively with practice team members and external agencies to meet patient needs.

Additional Responsibilities

Responsibilities may include:

  • Providing coverage for staff absences as needed.
  • Participating in team meetings and practice discussions.
  • Complying with statutory requirements related to safeguarding and reporting concerns.
  • Maintaining compliance with the Information Governance Compliance Toolkit.

Confidentiality

In the course of your duties, you will have access to sensitive patient information, which must be treated with the utmost confidentiality in accordance with practice policies and procedures.

Health & Safety

You will assist in promoting health, safety, and security within the workplace, including:

  • Utilizing personal security systems as per practice guidelines.
  • Identifying and managing potential health risks.
  • Applying infection control measures in line with local and national guidelines.
  • Maintaining a safe and hazard-free work environment.
  • Reporting identified risks promptly.

Qualifications

Essential:

  • Completion of a two-year accredited pharmacy technician program, combining practical experience and academic study.
  • Possession of a Level 3 NVQ Diploma in Pharmacy Service Skills and a Level 3 Diploma in Pharmaceutical Science.

Desirable:

  • Completion of the CPPE Pathway is preferred.

Experience

Essential:

  • Demonstrated experience in a patient-facing role.
  • Prior pharmacy-related experience beyond formal qualifications.
