As a Care Co-ordinator, you will be part of an inclusive and friendly team, supporting our clients in their own homes and making a difference where possible.

You will mentor your team setting goals and targets, help with overall health and safety, and assist the Senior Management Team in creating support plans.

You will contribute to developing novel strategies to assist individuals in reaching their own personal goals by taking a person-centred approach.


Job Opportunity
  • Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellbeing.
  • To ensure that all staff contribute to the efficient running of Harmonize Care Ltd and supervise the team.
  • To organise and coordinate care delivery in accordance with current best practices according to policy and procedures, agreed standards, legislative requirements, and relevant regulations under the direction of the manager.
  • To supervise the care delivery within the service
  • To undertake training and development required for your role.
  • Meet and greet clients and build relationships
  • Ensuring care plans are managed and Reviewed as needed.
  • On - Call service (approximately every 12 weeks)
  • The role will require you to be office-based as well as provide care in the community, supervising a team.

Essential Skills
  • To hold an NVQ level 3 or be willing to work towards it.
  • At least 2 years of experience at senior level.
  • knowledge of Access Care planning/People Planner would be advantageous, although, training would be provided for the right candidate.
  • To provide leadership to care staff.
  • To supervise the care delivery within the service
  • Have knowledge of safeguarding and CQC standards.
  • To provide care following current best practices, according to policy and procedure, agreed standards, legislative requirements, and relevant regulations, under the direction of management.

  • Self-starter, with a high level of motivation

  • Keen to learn in a fast-paced environment and creatively solve problems

  • You need to be flexible with your availability, being able to commit to early/late shift patterns and every other weekend working.
  • On-call service every 12 weeks (roughly)
  • Driver and access to a vehicle is essential due to the location of our clients.
