**About the Role**:
Colne Valley PCN has an opportunity for two care co-ordinators to join our primary care network (PCN), to support our 6 GP practices across the Colne Valley area to assist in re-shaping primary care across our communities. The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.

The care co-ordinator role is often the single most important role involved in the care of any patient. Supervising interdisciplinary care by bringing together the different specialists who provide the help that the patient may need, the coordinator is also responsible for monitoring and evaluating the support delivered, monitoring and coordinating patients' treatment plans, educating them about their conditions, connecting them with health care providers, and evaluate their progress.

The main duties of the role include:

- Supporting in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
- Supporting digital initiatives.
- Responsibility for the co-ordination of the patients' journeys through primary care.
- Auditing of data.
- Carry out telephone appointments.
- Carry out home visits, which means that a current driving licence and access to a car are essential to this role.
- Take part in multi-disciplinary team meetings.

If you are experienced in either a healthcare or admin background, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have have the following skills, experience and qualifications:

- Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
- Strong listening and communication skills;
- Familiar with local resources and services, including how to access them;
- Motivated by helping people, with care and empathy;
- Able to deliver person-centred support in a non-judgmental way;
- Experience of completing holistic person-centred care planning assessments;

**About Us**:
Colne Valley PCN is a semi-rural network of GP practices, serving approximately 55,000 patients across our 6 member surgeries: Coggeshall Surgery, Hedingham Medical Centre, Freshwell Health Centre, Kelvedon & Feering Health Centre, Pump House Surgery and Elizabeth Courtauld Surgery.

The PCN has become a limited company (14099411) and is led by a board of directors and shareholders who have oversight of the operations and finance of the PCN. All incoming PCN employees are hired via the PCN Ltd company and are provided with access to the NHS pension scheme, 6.6 weeks' annual leave which includes public and bank holidays along with appropriate CPD opportunities.

The PCN member surgeries & staff meet regularly, and are led in their work by a clinical director and supported by a dedicated PCN operations manager.
