Job summary
The post holder will be expected to provide clinical expertise and knowledge, demonstrating a sound understanding of the issues related to the identification, assessment, treatment, and management of patients at risk of developing cardiovascular disease (CVD). The post holder will support the development, running and evaluation of the newly formed CVD prevention service as part of the strategic partnerships team. They will manage the caseload referred to the service, monitor and improve standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. They will actively contribute to the clinical governance processes within the service.
Main duties of the job
To have responsibility for assessment, planning, development, implementation, and evaluation of individual programs of patient care within the CVD Prevention service.
Clinically assess patients referred with incidental findings or multiple risk factors of CVD in a clinic setting.
Assist in developing assessment tools, training packs and pathways in accordance with NICE guidance.
Act as a source of expertise and point of contact for the team, liaising with health care professionals, statutory and voluntary agencies by demonstrating highly specialised communication skills and problem-solving abilities when prioritising risk
Educate patients and their families about their condition

Job description
Job responsibilities
Liaise with the multi-disciplinary team to discuss patient care pathways
Liaise with the adult Familial Hypercholesterolemia / Diabetes and other specialist teams across the ICB region to provide a pathway for raising awareness
Work with primary care staff, regional consultants, physicians, nurse specialists and other related professionals to develop high levels of care for patients at risk of CVD
Actively engage with patients to obtain feedback from patient and family experience
Work as part of multidisciplinary team to identify and refer patients identified with a diagnosis of FH or diabetes to genetic counsellor if required
Undertake opportunistic diagnostic testing for the public and certain sectors using point of care testing. This will involve discussing information to inform and counsel individuals on the significance of such investigations both for the individual and their family members and relay information to the relevant healthcare professions
Provide advice and written information as required in response to clinic appointments.

Essential
1st level registration with NMC Registered Nurse Clinical examination qualification At least 2 experience within CVD speciality Evidence of commitment to continued professional development Degree level education or working towards or equivalent
