Job summary

An exciting opportunity has arisen to join the diabetes team at Liverpool University Hospitals NHS Foundation Trust based at The New Royal site.

The Clinical Nurse Specialists are part of a larger multi-professional team of Diabetes and Endocrine Consultants, Dietitians, Podiatrists, Psychologist and Admin team. This full time band 6 Clinical Nurse Specialist role in Diabetes will include community clinics, inpatient diabetes inreach, and diabetes outpatient clinics. There will also be an opportunity to support some of the speciality clinics.

Main duties of the job

You will provide care and self management plans to people with diabetes and also provide specialist diabetes management advice and support education to other healthcare professional colleagues, supporting events across the organisation around diabetes awareness and health and well being.

Full support through a diabetes competency programme will be provided

Job responsibilities

The role of the band 6 CNS in Diabetes includes the following: -

Perform specialist assessment of people with diabetes mainly in the inpatient environment, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills. Undertake the diabetes competency programme of education based on the national TREND Diabetes competencies. Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation with the support of the band 7 CNS and the wider MDT. Co-ordinate and manage patient reviews from the Diabetes Dashboard or other referral systems. Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements. Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery and to support safe discharge. To support, assist and encourage the multi-disciplinary team in the review of people with complex needs, by the provision of new and innovative models of case management. Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio in accordance with re-registration requirements. Participates in the supervision and the teaching of pre and post registration students, new staff members and junior medical staff. Providing in-reach education to clinical areas to support safe diabetes patient care. Contribute to the development of educational programmes in relation to diabetes care. Identifies training needs and develop training resources for nursing staff to enable role development and to meet the needs of the service. Take part in diabetes awareness programmes and national diabetes awareness activities to promote and support diabetes education and safe care across the organisation. Person Specification
