About the Role

We are seeking a highly skilled and experienced Diabetes Specialist Nurse to join our team at Mersey Care. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to patients with diabetes, promoting excellence in nursing practice, and contributing to the development of our services.

Main Responsibilities
  • Provide specialist nursing care to patients with diabetes, including assessment, diagnosis, and management of their condition.
  • Develop and implement individualized care plans to optimize patient outcomes and promote self-care.
  • Collaborate with other healthcare professionals to ensure seamless care delivery and effective communication.
  • Contribute to the development of our services, including the design and implementation of new care pathways and initiatives.
  • Provide education and training to patients, carers, and healthcare professionals on diabetes management and care.
Requirements
  • Degree or equivalent qualification in nursing or a related field.
  • Appropriate prescriber (V300) or enrolled on course and due to complete.
  • Evidence of post-registration experience in specialist area, including management and clinical leadership.
  • Registration with relevant professional body.
  • Clinical Examination and Diagnostics (or working towards).
