We are seeking a highly skilled and experienced Diabetes Specialist Nurse to join our team at The Diabetes Times. As a key member of our multidisciplinary team, you will play a vital role in driving up standards and quality in diabetes care.

Key Responsibilities:
  • Work autonomously within the field of Diabetes, utilizing evidence-based guidelines to deliver high-quality patient care.
  • Audit care given to patients and suggest improvements in services, ensuring seamless delivery of care across Acute, Community, and Primary Care settings.
  • Deliver complex educational packages to healthcare professionals and patients, promoting effective diabetes management and empowering individuals to take control of their health.
  • Act as a clinical expert and professional resource to Multi-Professional teams, patients, and carers, providing highly specialist advice and setting standards of care.
  • Contribute to achieving the objectives of the clinical service group/speciality corporate objectives, working within the clinical governance framework.
Requirements:
  • Experience working in Diabetes, with the ability to work autonomously and lead the development of specialist areas of practice.
  • Strong clinical expertise and ability to set standards of care, assess care needs, and develop, implement, and evaluate programs of care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with Multi-Professional teams, patients, and carers.
  • Ability to contribute to the development of specialist areas of practice, working in partnership with other members of the speciality teams across Acute, Community, and Primary Care settings.
