We have an exciting opportunity to join our Integrated Heart Failure Service at Liverpool University Hospital Foundation NHS Trust. We are seeking a highly motivated individual with excellent interpersonal and communication skills. The successful candidate will be responsible for the development of a cohesive management programme for patients with heart failure in line with local and national guidelines, working across acute, community and virtual ward settings. The service will operate 7 days per week and requires flexibility working within an evolving proactive, dynamic and committed multidisciplinary Heart failure Team

The main aim of the role is to identify patients across the two hospitals who can be supported on the Heart Failure Virtual ward, reducing hospital length of stay, preventing unnecessary admissions to hospital and improving Heart failure management and control with optimisation of evidence based treatments.

Essential elements of the role also include assessing, planning and the implementation of holistic packages of care to patients and their carer's who have a diagnosis of Heart Failure. Working as an autonomous non-medical prescriber you will provide community and domiciliary care including the timely optimisation of evidence based treatment and therapies.

The successful candidate will need to demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively, both independently and as a member of a team.

Main duties of the job

Working across the hospital with responsibility for patients with heart failure in the inpatient, outpatient day ward settings, community and virtual ward.

Responsible for the early identification of patients for the Heart Failure virtual ward from the acute setting and supporting the on going care of patients who are actively being manged on the Virtual ward.

The post holder will take overall responsibility to develop the specialist area of practice in partnership with other members of the speciality team ( Lead Clinician, Matron, Service Manager etc) will work collaboratively to develop services and quality of care post holder will be expected to provide first-line management and lead a team of specialist nurses (Sisters).This will be achieved through the core dimensions of the role, clinical practice leadership, acting as the patient advocate and through audit and post holder will carry responsibility for either a defined case load or offering "highly specialist service advise" this will include setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of post holder will contribute to achieving the objectives of the clinical service group / speciality corporate objectives and work within the clinical governance will lead the development and delivery of a programme of practice and professional development for staff within the speciality.

Admission prevention through care co-ordination identifying factors, which impact on quality of life such as, co-morbidities, psychological and social issues.

Symptom control across the continuum of care and advance care planning.

Perform specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical clinical care by managing a patient caseload providing expert assessment, planning and evaluation.

Ensure systems are in place for risk assessment and minimisation includingpromoting no blame cultures in order that near misses and incidents arereported.

Take the lead in investigation of incidents / complaints when required, including detailed reports / letters to complainants.
