Would you be interested injoining our friendly team of multidisciplinary healthcare professionals?Greater Middlesbrough Primary Care Network (PCN) is seeking a Clinical CareCoordinator to join our growing team.

The successful candidate will bepart of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT).Clinical leadership will be provided by the PCN Clinical Directors and ClinicalLeads.

The successful candidate will bebased in a local cluster of General Practices as part of the PCN.

We are looking for individualswho will lead clinical care home rounds& coordinate the work ofhealthcare professionals and non-clinical staff in the care of patientsregistered at GP practices within the wider PCN population. The role includesresponsibility to work with the PCN Operational Manager and the practices tosupport and ensure compliance with the reporting requirements of the PCNDirected Enhanced Services (DES).

Salary is negotiable dependingon experience and qualifications up to Band 7 AFC.

Main duties of the job

The role will involve workingacross organisational & professional boundaries to provide clinical careand leadership and co-ordinate the treatment, care, management, and empowermentof care home residents.

The individual will take part inthe weekly care home rounds to include

- Leadweekly ward rounds in our aligned Care Homes

- Weekly MDT's with our LearningDisability EHiCH colleagues in our LD Care Homes

- Review medication

The Clinical Care Coordinatorwill work closely with the Care Coordinators, GPs and practice teams to managea caseload of patients, acting as a central point of contact to ensureappropriate support is made available to them and their carers; supporting themto understand and manage their condition and ensuring their changing needs areaddressed.

SystmOne and EMIS clinicalsystems

Training practices

We are an active network andclosely work with our local voluntary organisations and other partners todevelop new models of care to benefit our patients.

The role will provide additionalflexibility to the PCN by adding to the growing expansion of the primary careworkforce.
