The ideal candidate will integrate into a vibrant and innovative PCN led by experienced Clinical Directors and Clinical Leads.

This position will be stationed within a network of local General Practices as part of the PCN.

We seek professionals who will spearhead clinical care home visits and synchronize the efforts of healthcare providers and administrative staff in caring for patients registered at GP practices within the broader PCN community. This role also involves collaborating with the PCN Operational Manager and practices to uphold and ensure adherence to the reporting obligations of the PCN Directed Enhanced Services (DES).

Compensation is negotiable based on experience and qualifications, up to Band 7 AFC.

Key Responsibilities

The position will require collaboration across various organizational and professional boundaries to deliver clinical care and leadership while coordinating the treatment, management, and empowerment of care home residents.

Responsibilities include participating in weekly care home visits, which encompass:

  • Leading weekly rounds in our affiliated Care Homes
  • Conducting weekly MDT meetings with our Learning Disability EHiCH colleagues in our LD Care Homes
  • Reviewing medication

The Healthcare Coordination Specialist will closely collaborate with Care Coordinators, GPs, and practice teams to manage a patient caseload, serving as a central point of contact to ensure that appropriate support is provided to patients and their caregivers, assisting them in understanding and managing their conditions, and addressing their evolving needs.

An exciting opportunity has emerged for a Healthcare Coordination Specialist to join our rapidly growing primary care team, serving the Greater Middlesbrough Primary Care Network (PCN), which supports over 70,000 patients across 8 practices in the region.

Greater Middlesbrough PCN comprises:

  • Multidisciplinary teams that currently include Clinical Pharmacists, Care Coordinators, Mental Health Practitioners, First Contact Physiotherapists, and Social Prescribing Link Workers
  • SystmOne and EMIS clinical systems
  • Training practices

We are an active network that collaborates closely with local voluntary organizations and other partners to develop innovative care models that benefit our patients.

This role will provide additional flexibility to the PCN by contributing to the expanding primary care workforce.

Experience Requirements

Essential

  • Proven experience working collaboratively at a senior level with health, social care, and voluntary sector providers, as well as primary and secondary care professionals, care homes, palliative care services, patients, caregivers, and commissioners
  • Experience in supervising and appraising staff
  • Background in working collaboratively with other organizations
  • Experience in clinical audit, evaluation research, and implementing evidence-based practices
