About the Role:
We are seeking a highly skilled and dedicated Care Coordinator to join our team at Randstad. As a Care Coordinator, you will play a vital role in supporting young people leaving care, helping them to navigate the challenges of independence and achieve their full potential.
Key Responsibilities:
- Case Management: Manage a caseload of young people leaving care, ensuring their needs are assessed and addressed.
- Pathway Planning: Develop and implement Pathway Plans that promote independence, well-being, and positive outcomes.
- Service Coordination: Support care leavers in accessing education, employment, housing, and other essential services.
- Collaboration and Communication: Collaborate with multi-disciplinary teams, external agencies, and other professionals to coordinate services and resources.
- Compliance and Governance: Ensure compliance with relevant legislation, policies, and procedures related to leaving care and child protection.
Requirements:
- Qualifications: Qualified Social Worker registered with Social Work England.
- Experience: Significant experience in a social work role, particularly working with care leavers or young people in care.
- Skills: Excellent communication and interpersonal skills, with the ability to build effective relationships with young people, families, and professionals.
- Knowledge: Knowledge of relevant legislation, policies, and procedures related to leaving care and child protection.
