Main duties of the job

The successful applicant will beregistered with the GPhC, and will undertake the CPPE pathway. They should bean independent prescriber or working towards, with strong written and verbalcommunication skills, an ability to work effectively under pressure and a goodunderstanding of the DES.

Job responsibilities

The Clinical Pharmacist will workwithin general practices providing practical and clinical support to improvethe management of medicines.

To undertake structuredmedication reviews to meet the PCN service requirements. Provide professionaladvice to health professionals about Medicines Management. They will beinvolved in educating patients and health professionals about medicines.

The post holder will work withintheir clinical competencies as part of a multidisciplinary team in a patientfacing role. The post holder will take responsibility for areas of chronicdisease management within the practice and undertake clinical medicationreviews to proactively manage patients with complex polypharmacy

The post holder will provideclinical leadership on medicines optimisation and quality improvement andmanage some aspects of the quality and outcomes framework and enhanced services

Principal Responsibilities:

Act as a pharmaceutical resourcefor patients, general practitioners, community pharmacists, nurses and otherICB staff

Review practice repeatprescribing policies and support the management of the repeat prescriptionre-authorisation process, by reviewing patient requests for repeatprescriptions and adherence of practice staff to the policy. Ensure patientshave appropriate monitoring tests in place when required.

Undertake face to face patientmedication review clinics in partnership with general practitioners and primarycare health professionals

Clinical

Plan, organise and undertakeunsupervised medication review clinics with patients in GP practices,particularly for vulnerable patients ( Those who are housebound, those whohave fallen, those in care home settings and those recently discharged fromhospital, those with long term conditions and polypharmacy)

Undertake medication review workin the most appropriate settings, which may include GP practices, care homes orintermediate care.

Provide general practitionerswith appropriate recommendations and actions plans following medication reviewclinics with patients to optimise the patients treatment with medicines, andensure appropriate follow up with patients

Record medication review plans,recommendations and outcomes on GP computer clinical systems and own clinicalnotes

Obtaining agreement with GPpractices to implement clinical review programs with the overall aim offollowing national prescribing guidance, improving the quality of prescribingand enhancing expenditure on medicines. Undertake changes to prescribing whereappropriate and agreement has been obtained

Analyse complex prescribing data(ePACT2) for practices to highlight areas where there is scope for improvingthe clinical effectiveness of prescribing

Promote good qualityevidence-based prescribing to meet national guidelines and targets NICEclinical guidelines and technology appraisals, antimicrobial stewardship

Identify cohorts of patients athigh risk of harm from medicines, through pre-prepared practice computersearches. This might include risks that are patient related, medicine related,or both.

Contribute to the development andreview of clinical guidelines and drug formularies

Contribute to the development ofservices that have medical components

Facilitate patient focusedmedication management programs to improve compliance and concordance

Liaise with local hospitals andcommunity pharmacists to ensure effective Medicines Management across theprimary/secondary care interface

Contribute to the effectiveimplementation of NICE guidance and implement changes to medicines that resultfrom MHRA alerts, product withdrawal and any other local and national guidance

Answer medicine related queriesfrom GPs and other health care professionals by interpretation of complexmedicines information and evaluating a number of options before communicatingan evidence based response to the enquirer

Reconciliation of medicines postdischarge

As a prescriber, to implement ownprescribing changes and tests as required

Work with the practice team toensure the practice is compliant with CQC standards where medicines areinvolved

Training and Education
