Thepostholder will operate within their own sphere of competence and agreedacceptable limits of practice to provide expert professional clinical advice topatients, carers and colleagues, ensuring clinical safety and excellence.

We actively participate in shared initiatives such as aLeg Club, Older Persons Team, Physician Associate Programme, First-ContactPhysio Team and home visiting service for acute housebound patients.

Our PCN comprises 4 GPPractices, delivering patient centred care to circa 30,000 residents. Byworking together in a Network, each Practice is able to offer a wider range ofservices, giving patients faster and more efficient access to the right care andsupport.

Clinical Practice

Work as part of a multi-disciplinary team to deliver safe and high quality care. Carries out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care. Delivery of care in line with NSF, NICE guidelines and evidence-based care The post holder will work within their clinical competencies to provide specialist pharmaceutical support to all practices of the PCN including working with members of the wider healthcare team to improve care and support safe and effective prescribing. Moving between two practices, the role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy. The post holder will need to be proactively involved in quality improvement and audit as well as managing some aspects of the Quality and Outcomes Framework. There will also be the requirement for the post holder to contribute to the provision of out of hours/extended hours/improved access services. The post holder will be supported to help develop the role and work to improve the care and outcomes for the patients across Devizes PCN. To play a pivotal role in the delivery of high quality primary health care within the Devizes PCN Making recommendation for/or prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.

Care Management

Communicates confidential and sensitive information to patients, relatives and carers in relation to their condition. Contribute to local and national practice targets clinical remit QOF, prescribing incentive scheme, National benchmarking. To monitor and meet care outcomes against standards and recommend or initiate changes as necessary. Clinical medication reviews for patients with single or multiple long term conditions where medicines optimisation is required. To contribute to the surgeries on the day offering if requiring pharmacist input. To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and to reduce risk of readmission. To be involved in multidisciplinary clinical meetings within the PCN where appropriate Contribute to the provision of out of hours/on call/extended services for the practice and the patients. These can include patient facing and telephone consultations, signposting to other services and/or healthcare professionals where appropriate, while working within scope of practice and limits of competency. To provide medication reviews four our patient groups in a variety of mediums including face to face, telephone and virtual. To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required. Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance. To maintain full and complete records of all patient contacts using appropriate clinical templates and coding To undertake a proactive role in audit and quality improvement implementing recommendations where appropriate. Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary. To identify areas of clinical risk at network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resources. To monitor network practices prescribing against the local health economy and make recommendations for GPs or amend within scope of practice, based on findings. To maintain own clinical and professional competence and be responsible for own continuous professional development. Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.

Administration and professional responsibilities

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures Participate in the administrative and professional responsibilities of the practice team Accurate and legible notes of all consultations and treatments are recorded in the patients notes Ensure clinical systems kept up to date, recording and/or amending accurate details Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice Attend and participate in practice and PCN meetings as required Restocking and maintenance of clinical areas and consulting rooms

Training and personal development

Maintain up to date skills and knowledge, maintaining awareness of professional issues at an advanced level. Training needs will be monitored by yearly appraisal and will be in accordance with the Practices requirements. Personal development will be encouraged and supported by the practice. Assess effectiveness of care delivery through self-reflection and peer review, as well as benchmarking and formal evaluation. Contribute to the identification and assessment of learning needs of staff and other professionals and assist in planning effective programmes of education. Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate. Act as a mentor for junior staff, assessing competence against set standards. Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work and ensure own educational commitment is at least sufficient to maintain revalidation requirements. Develop and maintain a Personal Learning Plan. Regularly participate in clinical supervision.

Leadership personal and people development

Act as a clinical leader in the delivery of primary care network clinical services ensuring that the needs of the patient are a priority Support staff development in order to maximise potential Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Critically evaluate and review innovations and developments that are relevant to the area of work Work with the network manager to ensure sufficient staffing to provide a safe service, and that staff have the appropriate ability, quality and skill-mix to meet current and future service delivery Take part in recruitment processes where appropriate. Maintain effective communication and active involvement in the planning and processes of practice-based commissioning or similar initiatives Promote the role of Clinical Pharmacist in general practice.

Management responsibilities

To assist the practice manager in design and delivery of new patient services In participation with the practice manager, assist with yearly appraisals of practice clinical staff and produce a training programme as appropriate. Develop and maintain network policies and protocols for all new and existing practice services To be the infection control lead for practice activity and ensure all associated staff are aware of current procedures Ensure agreed clinical audits are carried out in collaboration with practices clinical auditor. Ensure the practice manager is aware of any matters arising or problems / potential problems Ensure all patient information produced such as patient leaflets are up to date.
