About the Role
Clinical Pharmacist - Primary Care Network (PCN)

We are seeking a highly skilled Clinical Pharmacist to join our team in Devizes PCN. As a key member of our primary care network, you will play a pivotal role in delivering high-quality patient care and improving health outcomes for our community.

**Job Description:**
The successful candidate will be responsible for providing expert pharmaceutical support to our practices, working closely with healthcare professionals to optimize medication management and improve patient care. You will work autonomously within your professional scope of practice, providing face-to-face or telephone consultations, routine scheduled clinics, and structured medication reviews.

**Key Responsibilities:**
* Work as part of a multi-disciplinary team to deliver safe and high-quality care
* Provide specialist pharmaceutical support to practices within the PCN
* Conduct clinical medication reviews for patients with long-term conditions
* Reconcile medicines following hospital discharge and collaborate with community pharmacists to ensure smooth transitions of care
* Implement changes to medications in line with MHRA alerts and national guidance
* Maintain accurate records of patient contacts using clinical templates and coding

**Requirements:**
* Masters degree in pharmacy or equivalent at time of qualification
* Basic computer skills, including Microsoft Word and Electronic Patient Records (EPR)
* Knowledge of clinical governance and commitment to clinical supervision
* Up-to-date mandatory training certificates
