Kilmarnock, United Kingdom East Ayrshire Council Full time

Job Description

East Ayrshire Council where the people and the workforce are at the heart of everything we do.

East Ayrshire Health and Social Care Partnership (EAHSCP) has received additional funding to support and transform the way we care for adults and older people in our communities and is currently embarking on one of the biggest recruitment events undertaken for some time. Now is a really exciting time to join EAHSCP, to be part of this transformation and influence how we deliver support to those in our communities.

East Ayrshire has an excellent track record of zero delayed discharges that we continue to maintain and now we want to focus more of our attention on early intervention and prevention and supporting long term conditions at home. We are on an exciting journey of establishing Multidisciplinary Teams across our communities and work alongside our partner agencies to ensure everyone we support has access to the best preventative care that maximises independence.

To help us succeed, we are looking for a number of motivated, enthusiastic and creative Social Workers to extend our existing teams. We have vacancies within the Adult Concern Initial Response Team (ACIRT) and our Locality Health & Care teams. These teams are located across East Ayrshire as we aim to ensure that there is equity in the services that we provide, whilst recognising that by working in a collaborative way our teams are able to build strong, supportive relationships and deliver positive outcomes for those in our communities.

There are several vacancies within our 3 locality social work teams. Within these teams we protect and promote the welfare and wellbeing of Adults at Risk and those in our communities through the exercise of Statutory Powers to address complex issues. The team undertakes assessments of need, they formulate and implement care plans in line with National Self Directed Support Framework, to ensure that adults remain in their homes for as long as feasibly possible with the correct supports in place. The Locality teams work in conjunction with other external agencies and partner organisations in order to ensure positive outcomes for adults and with the promotion, delivery and integration of the key objectives identified within the Health & Social Care Partnership Plan and the Community Plan.

There is one vacant Social Worker post within ACIRT. ACIRT specifically respond to Adult Concern and Adult at Risk referrals for adults aged 16 years and over who are not an open case to social services. The work undertaken will often proceed to statutory work under the Adult Support and Protection (Scotland) Act . This is work that when proceeding to full investigation under this act requires to be conducted by a qualified Social Worker with Council Officer status. ACIRT act as the lead worker/team until the conclusion of the ASP Initial Case Conference. Due to the volume of referrals received there is an expectation that ACIRT workers assess, prioritise, and formulate an immediate effective response with an element of autonomy to ensure the safety and wellbeing of the individual they are working with. It is therefore preferable that Council Officers within ACIRT have substantial experience. ACIRT work in partnership with the multi-disciplinary team to ensure individuals receive the further assessment and support they require going forward.

This is an exciting time for EAHSCP and we are looking for motivated, creative and dynamic individuals who share our passion to provide caring, high quality support to the people in our communities.

If you are interested in this exciting opportunity and require further information, please contact Joanne Stevenson, Team Manager on
