We are looking for a caring, motivated, friendly Community Nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.

Ability to travel independently to fulfil the requirements of the post is essential.

Main duties of the job

To have a clear understanding of the vision of the Trust and be able to translate this into a local context for day to day administrative and operational implementation within a Community setting .

To assist the Caseload holders and Team Leader in promoting the vision and contributing to the establishment of an ethos of modern, customer orientated services and translates that into a culture of effective patient/user centred delivery.

The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.

We are committed to clinical support, CPD & training.

The post is covering 7 days 8am to 6pm service, on a Rota System.
