Job summary

Navigation 1 is seeking to recruit Clinical Pharmacists to develop & support medicines optimisation work across the 11 GP practices within the Primary Care Network (PCN).

This is an exciting opportunity for a Clinical Pharmacist to join a developing PCN. The post will be directly employed by Navigation 1 but will work across a number of Practices and be supported by our GP practices Clinical Pharmacists who all have a wealth of primary care experience to support your development.

The post holder will work as part of a multidisciplinary team in a patient-facing role. The Clinical Pharmacists will take on responsibility for areas of chronic disease management and will work proactively with other healthcare professionals as part of this across the organisation and PCN area. Working alongside other Pharmacist and Technician resource within our practices and CCG. We are keen to hear from pharmacists of all backgrounds and level of experience, whether you have already completed or would like to complete the independent prescriber qualification. Full time working will be considered

Main duties of the job

The PCN ClinicalPharmacist will need to be committed to improving health and putting patientsfirst. Key qualities will be the ability to build effective relationships withpatients, GPs, nurses, community Pharmacists, hospital and practice staff, andto work effectively as part of a team.

The post holder will:

Work as part of a multi-disciplinary team in a patient-facing role Support delivery of the new network Directly Enhanced Service (DES) Conduct structured medication reviews to proactively manage patients with complex polypharmacy Provide clinical medicines expertise to both patients and practice teams Improve prescribing practices across the PCN Improve service and quality through mechanisms such as clinical audits Interface with community and hospital colleagues to help resolve medication-related issues Assist with delivery of enhanced health in care homes initiatives alongside the already established CCG Care Homes Medicines Team.

It is a requirement of therole to be registered with the General Pharmaceutical Council. Primary careexperience would be an advantage however is not essential. Flexibility,enthusiasm and good clinical knowledge are essential.

About us

Coventry Navigation 1 PCN consists of 11 GP Practices covering approx. 80000 patients. Our network operates with in the city of Coventry.

The successful candidatewill join a core PCN Pharmacy Team and will be supported by a Senior ClinicalPharmacist and a GP Supervisor. Regular development sessions, teaching,training and opportunities to upskill will be provided.

KEY DUTIES ANDRESPONSIBILITIES

Clinical Duties

• Performface to face medication reviews of patients with polypharmacy, in practice andin patient’s homes (including care homes).

• Throughstructured medication reviews, support patients to take their medications toget the best from them, reduce waste and promote self-care

• Provideexpertise in clinical medicines review and address public health and socialneeds of patients in GP practices 2

• Discusscomplex and sensitive information with patients during medication reviewincluding medical histories, lifestyles and personal circumstances and to referon to other services as appropriate.

• Improvepatient and carer understanding of, confidence in and compliance with theirmedicines.

• Provideleadership on person-centred medicines optimisation (including ensuringprescribers in the practice conserve antibiotics in line with localantimicrobial stewardship guidance) and quality improvement whilst contributingto the quality and outcomes framework and enhanced services.

• Providegeneral practitioners with appropriate recommendations and actions plansfollowing medication review clinics with patients to optimise the patient’streatment with medicines and ensure appropriate follow up with patients.

• Recordmedication review plans, recommendations and outcomes on GP computer clinicalsystems and own clinical notes in accordance with practice procedures.

• Reconcilemedicines following discharge from hospitals, intermediate care and into carehomes; identify and rectify unexplained changes; manage changes by referral tothe GP; perform clinical medication review; support the production of post-‐discharge medicines care plans and work with patients andcommunity pharmacists to ensure patients receive the medicines they need.

• Supportthe processing of complex repeat prescribing medication requests made bypatients or other repeat prescribing queries.

• Work inpartnership with hospital clinicians to proactively manage patients at highrisk of medicine-‐relatedproblems before they are discharged to ensure continuity of care.

• Liaisewith the GP and Nurse Practitioner team on prescribing and medication issues,especially around management of long-term conditions.

• Developas an independent prescriber to manage and prescribe for patients within aspecific chronic disease area.

• Withappropriate training, monitor the effects of medication by checking clinicalsigns ( BP, pulse monitoring, spirometry, etc).

• Attendregular clinical supervision sessions to promote a culture of continuousprofessional development and shared learning.

SupportiveClinical Duties

• As partof the multi-disciplinary team, attend relevant MDT Network meetings, providinginformation and feedback.

• Reportmedicines related incidents, contribute to investigations and root causeanalyses as well as participating in serious incident investigations and multidisciplinarycase reviews

• Delivertraining, mentoring and guidance to other clinicians and staff on medicineissues.

• AttendProtected Learning Time sessions (PLT’s) and possibly practice awaydays/development sessions.

• Ensurethat, within each practice you work, staff and clinicians are kept up to date regardingchanges in national and local medicines guidance and legislation.

• Criticallyappraise clinical evidence and present information to prescribers viaprescribing meetings and educational events, as required.

• Lead onmedicines management governance arrangements to adhere to CQC standards,including assisting in developing policies and SOPs.

• Review repeat prescribing protocols andprocesses and support staff in their implementation

• Perform prescribing audits and direct/supervisethe audit activity of others

• Facilitate good working relationships betweenpractices and local community pharmacists.

• Be responsible for delivering local prescribingquality initiatives in collaboration with CCG medicine management.

• Develop an effective network and workingrelationship with individual practice GPs, practice staff, CCG medicinesmanagement colleagues, secondary care and other professionals.

• All members of staff have an individualresponsibility to comply with the organisation’s policies and practices.
